Summary of Position:
Circle the City is a rapidly growing non-profit dedicated to providing high-quality, holistic healthcare to people experiencing homelessness in Maricopa County. This clinic benefits from the designation as a Federally Qualified Health Center (FQHC) and provides comprehensive primary care alongside fully integrated behavioral health and case management services to those experiencing homelessness each year.
The Medical Coordinator assures the continuity of client care, providing an essential link for communication involving all team members. Assists in the examination and treatment of patients under the direction of the physician and/or nurse manager. This is a safety-sensitive position.
Essential Duties:
Duties include, but are not limited to:

Collaborates with clinical support team each shift to determine work assignments and priorities, as well as communicate the status of delegated task completion and outcome of patient data findings.

Maintains timely, ongoing verbal communication to ensure good care of patients and the effective running of the clinic.

Responsible for collecting, collating, and entering required data and information into the applicable database or format in an accurate and timely manner.

Treats each patient in a manner that preserves and supports patient confidentiality, patient rights, and patient dignity.

Checks inventories on a weekly basis, orders, and stocks supplies.

Assist any patient with personal hygiene as needed.

Assures a clean, orderly, and functional work environment.

Escorts patients to off-site appointments as assigned by supervisor.

Facilitates patient transportation.

Assists with copying, faxing, and filing as needed.

Phone coverage and triage of calls when necessary.

Ability and willingness to perform medical assistant duties at mobile clinic sites.

Performs other duties as required.

Obtaining vitals, administering vaccinations and medical injections, basic wound care, call on prescriptions, drawing blood, process lab specimens, etc.

Collect, verify, and register new patients, scan records to chart, etc.

Process referrals, call for appointments, schedule transportation, request consult records, etc.

Provides community resources, and assists with connecting to support services.

Obtains vital records for patients, provides, and assists in housing applications, networks within the community for housing options, transports patients to locations for housing assistance/applications.

Drives the Mobile Medical Unit, transports patients to the lab, imaging, pharmacy, and appointments.

Qualifications:
Basic Knowledge and Skills:

Basic knowledge of medical terminology.

Completion of a Medical Assistant program, and/or alternate allied health training program deemed appropriate by the CMO, Medical Director, and Chief Nursing Officer to provide competency in the skills necessary to complete the essential duties noted within this job description and must have a minimum of one (1) years of related experience that demonstrates an understanding of the required knowledge, skills, and abilities.

Able to obtain vital signs accurately.

Ability to use personal computer for recording patient data /history.

Proficient in reading and writing skills in English.

Excellent communication and customer service skills.

Ability to work in a fast-paced, challenging environment.

Demonstrated interest in working with an underserved population.

High school diploma or GED required.

Bi-lingual communicative skills preferred

Basic Life Support (BLS) certification.

Requires a current Arizona state driver’s license.

Must be at least 21 years of age.
Physical and Mental Requirements:

Position requires extended periods of sitting and standing.
-
Position requires regular bending and reaching, including transfer of patients.

Must be able to lift, carry and push 20 pounds on a regular basis.

Must be able to lift and carry up to 50 pounds on an occasional basis.
