$10,000 Signing Bonus! Divided into 3 payments at 90 days, 6 months, and 1 year
Summary of Position:
Each patient that is referred to the Physical Therapy Program by a CTC physician will receive a complete evaluation by a licensed physical therapist. Each patient will then receive treatment from a licensed physical therapist and/or physical therapist assistant according to an individualized treatment plan. All services are to be recorded in the Electronic Health Record (EHR).
Essential Duties:
Duties include, but are not limited to:
- Treat assigned patients according to plan established by supervising physical therapist
- Document each visit in the Electronic Health Record (EHR) within 24 hours of treatment
- Document and discuss progress toward established goals with supervising physical therapist
- Participate in team discussion surrounding patients’ plan of care, goals and continued therapy
- Communicate with supervising physical therapist in person or by phone
Qualifications
Qualifications:
Basic Knowledge and Skills:
- Understanding of substance and mental health issues required.
- Interpersonal and communication skills sufficient to effectively interact with patients, physicians, and health team members required.
- Computer proficiency inclusive of ability to access, enter, and interpret computerized data/information required.
- Basic Life Support certification required (BLS).
- Associate degree from accredited program required
- Arizona Physical Therapist Assistant license with ability to work under general supervision required
- Must be at least 21 years of age.
