Valid Driver’s License
Social Security Card
Signed Insurance Check Authorization Form
Signed Criminal Background Check Authorization Form
10-Year DMV Record
Ability to Pass a DOT Physical & Drug Screening
Training Provided by circYoulation:
First Aid & CPR Certification
Defensive Driving Course
PASS Hands-On & Online Training
HIPAA Compliance Training
Driver Responsibilities:
Be punctual, reliable, and professional at all times
Drive long distances safely
Start early morning shifts (as early as 3 AM)
Own an Apple or Android smartphone
Maintain flexibility to meet dispatch needs
Perform fluid checks & report vehicle maintenance
Keep the vehicle clean & sanitized
Communicate effectively with dispatch
Use GPS & company apps/software
