Description

The CCBHC Psychosocial Team Lead (TL) is a multi-function position that will coordinate the mental health/substance use recovery needs of individuals in the outpatient setting of the Certified Community Behavioral Health Clinic (CCBHC) within the Seaway Valley Region (SWV). They will also analyze and track data within the SWV region to increase productivity and the quality of care provided. The goal is to improve linkage to care, promote mental health and well-being. The CCBHC Psychosocial Team Lead (TL) will act as a subject matter expert in CCBHC services to promote and maximize person-centered care through the promotion of integrated convenient care, enhancing communications between care providers and monitoring outcomes.

Essential Functions (Job Duties):

Team Lead Responsibilities:

  1. Provides daily direction and communication to employees.
  2. Provides continual evaluation of processes and procedures. Responsible for suggesting methods to improve area operations, efficiency and service to both internal and external customers.
  3. Provide coaching to staff on CCBHC services and the integrated care model of providing integrated convenient care through combined offerings of services in a single day and track data associated with combined offerings using CCBHC Threshold data.
  4. Provides performance feedback and coaching on a regular basis to each team member.
  5. Is available for employees who experience work and/or personal problems providing appropriate coaching, counseling, direction and resolution.
  6. Ensures employees have appropriate training and other resources to perform their jobs.
  7. Creates and maintains a high-quality work environment so team members are motivated to perform at their highest level.
  8. Establishes work procedures and processes that support the company and departmental standards, procedures and strategic directives.
  9. Uses appropriate judgement in upward communication regarding department or employee concerns.

Outpatient Responsibilities

  1. Working to secure Medicaid and other relevant benefits.
  2. Interviewing and screening for individual’s needs and program eligibility.
  3. Assisting individuals in maximizing community integration and normalization.
  4. Providing leadership in the coordination of resources.
  5. Referral of individuals to community and social support services.
  6. Transitional Care and linkage, including follow-up from inpatient and other settings.
  7. Provide support in navigating complex health care and social service systems.
  8. Provide individuals and families support toward goals.
  9. Provide advocacy for needed services.
  10. Provide on-site crisis intervention and skill teaching when other services are not available.
  11. Monitor and follow up contacts and other activities (at least one annual monitoring).
    1. To ensure services are consistent with the individuals’ treatment plans.
    2. To ensure services are adequately addressing individuals’ needs and wellness recovery goals.
  12. Monitor progress by collaborating with:
    1. The individual.
    2. The individual’s family or collaterals.
    3. Other relevant entities or individuals.
  13. Consulting with collaterals, with clients’ permission, with the goal of improving the individuals’ outcomes while assisting them in addressing their goals.
  14. Taking the lead role when collaborating with service providers in a coordinated effort to ensure continuity of care for the individual.
  15. Preparing and maintaining consumer records in accordance with departmental procedures.
  16. Developing other services to meet clients’ needs when no appropriate services exist, or when existing services are inadequate.
  17. Reporting to supervisor/manager/director for all emergencies and other concerns and work toward a resolution.
  18. Other duties as assigned to meet the needs of the consumers and department.
  19. Adhere to agencies policies, procedures and SOPs.
  20. Maintain all required documentation in a timely manner.
  21. Teach and strengthen daily living skills
  22. Assist in navigation of transportation.
  23. Assist in developing healthy coping alternatives especially to anger and challenging situations.

Qualifications


Required Education and Experience:

  1. High School Diploma or equivalent.
  2. Work with mental health or substance use population preferred.
