JOB SUMMARY
A Case Manager has the responsibility, accountability and authority for providing comprehensive case management for patients. Responsible for daily patient care coordination with other members of the healthcare team, assessment of patients' level of care needs, and communication of patient's needs to the other providers regarding discharge planning. Serves as liaison and assists in the resolution of issues that cross multiple departments and/or services to ensure optimal quality, cost and service/outcomes.
Job Specific:
- Assesses and evaluates patients discharge planning needs and documents timely in the patient’s Medical Record. (EF)
- Communicates timely with patients, physicians and nurses to identify post discharge services needed by patients.
- Identifies issues that may delay patient discharge and facilitates timely resolution of these issues. (EF)
- Develops and documents a post discharge care plan in conjunction with other members of the health care team.
- Coordinates care post discharge with physician, insurance carrier, post-acute providers, patient and family. (EF)
- Provides medical information appropriate to establish smooth transitions of care.
- Collects data about patient's progress and modifies discharge plans based on changes in patients' plan of care.
- Completes all documents required by payors and regulatory agencies timely and accurately. Identifies and manages – Resource Management issues. (EF)
- Assists with orientation of new personnel.
- Completes timely Utilization Review (level of care criteria reviews and insurance reviews). (EF)
Hospital:
- Adheres to organization-wide and other applicable policies and procedures.
- Day-to-day performance complies with the hospital’s Service Excellence Behavioral Expectations.
- Performs within the prescribed limits of the hospital’s Ethics, Compliance and Confidentiality Program guidelines.
- Performs within professional Scope of Practice at all times.

EDUCATIONAL REQUIREMENTS
Required: Associate’s Degree in relevant field
Preferred: Degree in Social Work or Nursing, Bachelor of Science degree in Nursing
EXPERIENCE
Required: Hospital or healthcare experience
Preferred: 1-2 years of hospital case management experience
SPECIAL SKILLS AND ABILITIES
Required: Effective verbal and written communication skills, direct patient care experience, or Social Worker experience.
Preferred: Healthcare reimbursement knowledge
LICENSURE / CERTIFICATION REQUIREMENTS
Required: RN, LVN or LSW (other relevant healthcare registries/licenses/certifications will be considered)
Preferred: LMSW, CPCM, CPUR, CCM
