What are the responsibilities and job description for the Care Coordinator position at Citizens Memorial?
Citizens Memorial Hospital (CMH) is recruiting a Care Coordinator for our Care Coordination Department to assist patients in the Osceola, MO service area.
CMH Care Coordination provides support and assists patients with navigating the healthcare system and facilitate care both inside and outside of the patient’s primary care clinic to improve patient outcomes.
A typical day will include: Serving as the in clinic resource for facilitating care needs outside the clinic; working closely with patients, providers, and other staff to schedule appointments and obtain authorization for primary or specialty appointments and diagnostic testing; tracking information regarding appointments and admissions outside the medical home, and communicating status or delays to the care team. The clerk will also ensure that the record of care that occurs outside the EMR is received and added to the patient’s electronic medical record (EMR).
The position is: Full-time Days. Monday-Friday 8am-4:30pm.
