Citizens Memorial Healthcare Foundation (CMH) is recruiting a Social Services Designee with additional duties as a Domestic Care Technician for our residents in Ash Grove Healthcare Facility
The skilled nursing facility is a 82-bed facility located in Ash Grove, MO that includes a memory care unit for individuals with Alzheimer's disease and dementia.
A typical day will include: Providing Social Services to residents, including staying with residents throughout their stay at the facility and you will be responsible for assisting with completion of the admission process and working with the interdisciplinary team on discharge planning. In addition to social services responsibilities, this role will be a Domestic Care Technician in a Per Requested Need capacity, which includes knowing and applying current policies and procedures in the care of residents; maintaining a safe, and clean long term care environment. You will assist in serving food trays, transporting residents to the dining room, and restocking linens in resident rooms.
Job Requirements:
- Education: Bachelor’s degree in Social Work or related field required
- Experience: NA

- Licensure/Certification: NA
- Physical: Ability to stand 30% of the day and work at a computer for the remaining 70%
The position is Full Time, Days including duties of both the Social Services Designee and Domestic Care Technician.
