Citizens Intermediate Care Facilities (ICFs) provide support and services for people with intellectual and other developmental disabilities who have multiple cognitive, medical and/or physical disabilities requiring 24-hour support and supervision. In this vital role, you will ensure the medical care of all people in the facility by providing direction to nursing and AMAP staff, completion of all regulatory reports, negotiating with outside agencies and acting as a liaison between hospitals, clinics, and consultants to ensure accurate coordination/delivery of services.

To qualify for the LPN role, you’ll need:
- NYS Licensed Practical Nurse
- CPR, fire safety training, and medication knowledge
- Previous experience supporting individuals with intellectual and other developmental disabilities is preferred.
