What You’ll Do :
- Make an Impact : Provide hands-on therapy and personalized care to help our residents achieve their highest level of independence.
- Innovate & Educate : Recommend home modifications, adaptive equipment, and provide training to improve daily living.
- Collaborate : Work with an interdisciplinary team of doctors, nurses, and therapists to create tailored care plans that maximize patient outcomes.
- Be a Resource : Educate residents, families, and caregivers on treatment plans, adaptive techniques, and progress.
- Lead with Excellence : Ensure all therapy is delivered in accordance with the established plan of care and meet state and federal regulatory requirements.
- Monitor & Adjust : Assess patient responses to treatment and make modifications to the therapy plan to achieve optimal results.
