Community Habilitation (Formerly known as At-Home Residential Habilitation) is a waiver service approved in OPWDD's Home and Community Based Services (HCBS) Waiver. CH is a goal-oriented service that provides assistance in developing and improving skills that will help an individual increase their functioning. This program aims to build skills that will increase independence in their home as well as providing opportunities for community inclusion. This service is provided in the community where the person lives in order for them to develop relationships in their own neighborhood. Here at City Access we largely focuses on skills building in the home. We work to improve household and personal care skills at every level. We strive for everyone to become as independent as possible in the management of their own home life. We encourage individuals to take responsibility for themselves, their space and their belongings. Community Hab is an excellent way of giving individuals with disabilities who live at home the ability to partake in a more well-rounded life experience that will carry over to all areas of their lives. While this program generally operates during afternoon/evening hours and on the weekends, City Access New York Community Hab department does its utmost to provide families with staff according to their specific needs and schedules.
Responsibilities:
- Address Community Habilitation Plan valued outcome(s), staff services and supports, safeguards and executing and maintaining all goals identified in individuals Hab Plan.
- Assess the individual’s interests, abilities, strengths, limitations, development, individual needs and overall functioning on an ongoing basis
- Enhance the individual’s strengths, capabilities and overall independence
- Enhance social skills and opportunities for socialization
- Provide assistance and/or total support with Activities of Daily Living Skills (ADLs)
- Perform light housekeeping tasks
- Ensure close One-to-one (1:1) supervision of the individual at all times
- Foster and promote community inclusion and integration
- Provide assistance and/or total support to establish and maintain schedules and routines
- Identify and/or provide opportunities for exploration and new experiences
- Promote a healthy lifestyle and healthy living; healthy diet, exercise, mental, physical and dental health and hygiene, social and emotional well being and health
- Maintain frequent communication while working collaboratively with the individual’s circle of support
- Must comply with EVV check in/out.
- Complete required daily documentation day and date with service provision.
- Maintaining accurate and organized records of individuals information and Community Habilitation Plan, sharing with designated individuals and/or providers as necessary.
Skills/Experience:
- Must have excellent customer service skills to develop and maintain productive relationships with participants, families, employers, participant co-workers, volunteer community sites, schools, transition coordinators.
- Special populations and/or projects will require creativity and prior related experience.
- Ability to communicate effectively both orally and in written form.
- Ability to address groups for both training and community education purposes.
- Ability to organize work.
- Ability to establish and maintain an effective working relationship with participants’ families, employers, funding sources, Medicaid Services Coordinators, etc.
- Ability to counsel participants both individually and in groups and develop treatment goals based on participant’s needs.
- 1-2 years’ experience working with individuals with disabilities.
- Experience job coaching for individuals with disabilities preferred.
Requirements:
- High school diploma or G.E.D.
- Experience working with adults with disabilities and experience working in vocational programs preferred.
- Flexible hours are required.
- May be asked to work weekends and hours between 6 am. – 12 am.
