Job Summary:
City Best Hospice Care is seeking a compassionate and dedicated Hospice Chaplain to provide spiritual support to patients and their families during end-of-life care. The Chaplain will offer direct spiritual counseling, coordinate care with community spiritual counselors, and provide emotional and religious support in alignment with the patient’s beliefs and needs.
Key Responsibilities:
- Conduct spiritual assessments for patients and families, ensuring timely and appropriate services.
- Develop and implement individualized spiritual care plans as part of the comprehensive hospice care plan.
- Provide direct spiritual and emotional support to patients, families, and caregivers.
- Serve as a liaison to community chaplains and spiritual counselors.
- Maintain records of spiritual care services for quality assurance and program development.
- Document all services provided and coordinate with community clergy as needed.
- Participate in interdisciplinary team meetings and collaborate on patient care plans.
- Educate and support hospice staff regarding spiritual care practices.
- Recruit and coordinate community chaplains, spiritual counselors, and volunteers to meet patient needs.
- Conduct funeral or memorial services upon request
Qualifications:
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) or evidence of completion of three or more units of Clinical Pastoral Education.
- Experience working with death and dying, individuals/family/caregiver.
- Ability to work as member of interdisciplinary group.
- Comfort in an interfaith setting.
- Ability to accept different lifestyles, cultures, beliefs, and values.
- Ability to network with community clergy and congregations.
- Knowledge of and commitment to hospice philosophy of care.
- Hospice/ experience preferred.
- Have reliable transportation and willing to commute to provide the best in home patient care
Job Types: Part-time, Per diem
Pay: $40.00 - $45.00 per hour
Expected hours: 20 – 30 per week
