Key Responsibilities
- Deliver comprehensive case management and supportive services to clients in various settings.
- Facilitate connections between clients and appropriate housing and supportive services.
- Collaborate with a network of stakeholders including nonprofits, government agencies, and healthcare providers to ensure holistic care.
- Maintain accurate and timely data collection and reporting.
- Engage with clients directly in the field as well as in diverse environments such as local neighborhoods and shelters.
- Drive a company vehicle as necessary to transport clients to appointments and services.
- Minimum of 1 year of case management experience with targeted populations.
