An Overview
The Human Services Specialist I / Clinical Case Manager position in our Jail Behavioral Health Services Team provides supportive counseling, clinical case management services, care coordination, outreach, advocacy, and an interdisciplinary teamwork approach in service delivery. This position is also responsible for prioritizing and scheduling psychiatric services for incarcerated individuals who are referred from therapists on the Behavioral Health Team. They work closely with the jail medical contractor and outside pharmacy providers to coordinate care and ensure that medication orders remain uninterrupted. The population served are adult clients with serious mental illness diagnoses and/or co-occurring psychiatric and substance use diagnoses. The Clinical Case Manger provides support and assists clients in utilizing available resources to meet their personal, social, and medical needs, while empowering clients to self-determine their goals for treatment and achieve independence and self-reliance.
What You Should Bring
The ideal candidate should have thorough knowledge of behavioral health and substance use disorders; good knowledge of available community resources, such as medical, legal, mental health and social services; and good knowledge of treatment issues related to behavioral, emotional and social problems, including dual diagnosis, and substance use disorders. Good knowledge of group dynamics and commonly practiced approaches to counseling; good knowledge of psychopharmacology as it relates to the properties, actions, and health effects of illicit drugs, and commonly prescribed anti-anxiety, anti-depressant, and anti-psychotic medications. Excellent administrative and organizational skills.
The Opportunity
- Facilitates the Psychotropic Medication clinic, including scheduling clients and ordering medications;
- Assesses clients, provides information and referral services and ensures they are linked to appropriate departmental and community resources;
- Consults with other professionals and collaterals regarding clients’ needs;
- Collaborates in interdisciplinary meetings to assist with triaging referrals and workflow;
- Leads motivational and psychoeducational groups based on manualized treatment models;
- Provides short term supportive counseling, crisis intervention, recovery coaching, and other evidence-based techniques;
- Maintains clinical records in accordance with State, City, Departmental, and Federal compliance requirements and administers standardized evaluation instruments;
- Develops and updates consumer treatment plans in collaboration with clients and their treatment team;
- Maintains communications with various agencies and courts to coordinate services and provide reports on consumer progress;
- Other related duties as assigned.
About the Department
The Center for Adult Services (CAS) provides programs and services in Aging; Adult Protective Services; Clinical & Emergency Services; and Community & Residential Support Services for seniors and for persons with mental illness, intellectual disabilities and substance use disorders. CAS provides compassionate and effective person-centered services based on best practices. Our teams support self-determination, safety and recovery for Alexandria residents affected by abuse, neglect, mental illness, intellectual disabilities and substance abuse disorders.
Associate's degree in sociology, psychology, or a related human service field; one year of experience in case coordination, teaching or counseling the economically disadvantaged/developmentally disabled, and emotionally/physically abused/neglected children's population, local offender population, or a homeless population; valid driver's license issued by the state of their residence; or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.
Fluent and Bilingual in both English and Spanish; Master's Degree; At least three years of experience in the provision of mental health and /or substance use treatment services to persons with serious mental illness; At least one year of experience working with clients in crises and engaging non-compliant individuals in treatment; Currently licensed in the State of Virginia as an Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) or Clinical Psychologist.
This position requires the successful completion of pre-employment background checks including but not limited to: FBI/Federal Records Check; VA State Child Abuse/Neglect Registry; Medicare/Medicaid Fraud Database; education/certifications; and driver’s license.
Schedule: Monday through Friday, 8:00AM - 4:30PM.
Salary : $49,149 - $65,836
