Public Health Nurse II - Communicable Disease/Long-Term Care

An Overview 
The Public Health Nurse II (PHN II) is a member of an interdisciplinary team working on Population Health initiatives related to clinical services and long term care and works under the direction of a Public Health Nurse Supervisor. This PHN primary responsibility will be to provide nursing support to the DMAS-LTSS screening program including coordination with DCHS social workers to complete Universal Assessment Instruments using ePAS web-based data entry system. This position will require field/home visits. This PHN will also provide support to our clinical services for Immunizations, Sexual/Reproductive Health as well as responding to public health emergencies.

The Opportunity 
  • Works with DCHS social workers to conduct screenings of adults and children requesting community-based services and/or nursing home placement, including health history and system assessment identifying physical and psychosocial needs, and documents assessment utilizing ePAS program approved by DMAS;
  • Provides clinical nursing support to blended clinics including Sexual/Reproductive Health, and Immunization services;
  • Serves as agency liaison, providing education, training, and support to long-term care facilities in Alexandria to assist them with resources and strategies for mitigation of communicable disease outbreaks within their facilities;
  • Participates on assigned AHD activities such as committees, communicable disease prevention and emergency planning;
  • Prepares a variety of required/requested reports using Microsoft Excel, Word, Publisher;
  • Performs other related work as assigned/required.

This PHN will collaborate with develop partnerships and relationships with Department of Community and Human Services (DCHS) and Long-Term-Care Facilities throughout the City.

To qualify for this position, all candidates must possess a Four-year College Degree from an accredited college or university and/or graduate from an accredited professional school of nursing; three years of responsible public health nursing experience that include one year of experience providing services to individuals and families in a variety of settings such as community, clinics and schools; or any equivalent combination of experience and training which provides the required knowledge, skills and abilities. Eligibility for, or current licensure with the Commonwealth of Virginia as a registered nurse, and such other qualifications which may be prescribed by the Virginia Department of Health or any equivalent combination of experience and training which provides the required knowledge, skills and abilities. Employees must possess a valid driver’s license issued by the state of their residence.To be considered an ideal candidate, one should also possess knowledge of principles and practices of public health nursing, epidemiology, family dynamics, infection control measures, and health education strategies. Skilled in independent implementation of the nursing process, including assessment of client needs and providing appropriate interventions. Ability to function well with a great deal of autonomy in a variety of settings, and communicate effectively both orally and in writing. Ability to work as part of an interdisciplinary team. Knowledge and practice skills at the professional level of nursing such as, advanced physical and psycho-social assessment skills, knowledge of the vaccine preventable diseases and experience administering vaccines. Demonstrate work experience in outpatient health, communicable disease, family planning and sexually transmitted infection prevention and treatment practices and has progressively worked into clinic oversight positions. Demonstrates knowledge or experience providing health education and outreach to groups or individuals from diverse ethnic, social and economic backgrounds. Competent in Microsoft applications such as Word, Excel, PowerPoint and Publisher. Bilingual preferred (Spanish/Amharic/Arabic/Farsi). This position requires the successful completion of pre-employment background checks including but not limited to state and federal criminal history background check, driving record check, National Sex Offender Registry check, U.S. DHHS IG Exclusion report and GSA search.

Candidate must provide own vehicle for routine transportation between location.

Salary : $72,602 - $88,210
