SALARY : $19.71-$22.00 per hour, DOQSCHEDULE : Monday - Friday, 8-5This is a shared position between the Amarillo Public Library (APL) and Community Development Department. The position will report to the Coming Home Case Manager Lead in the Community Development Department. The Library Case Manager will work 20 hours per week with the Amarillo Public Library and 20 hours per week with the Coming Home team. The Library Case Manager provides strengths-based case management and service support. This position will help identify library patrons who may benefit from referral to community resources such as primary and mental health services, substance abuse services, educational resources, housing services, food and community resources, job readiness and placement, and assistance with securing vital documentation. Individuals will be identified either through patron request, individual patron need assessments or through direct referral by APL staff.The Coming Home program employs the Housing First approach to house and provide supportive services to those experiencing chronic homelessness. Honoring the inherent dignity of those the program serves, Coming Home affirms client capabilities and encourages self-sufficiency, giving hope that a better life is possible. This position is grant funded and renewal is contingent upon the availability of funds. Grant funded positions are typically limited in term. The end date of the grant does not guarantee employment for the duration of the grant. You may be eligible to apply / transfer to an existing vacancy that is equivalent to the grant funded position if an equivalent position is available.NOTE : Employees in the Community Development Department and their family members are prohibited from participating or receiving benefit from the City of Amarillo's programs funded by the Department of Housing and Urban Development and other state and federally funded programs and grants.ESSENTIAL RESPONSIBILITIES Identifies and engages with individuals who may benefit from social service support through outreach, direct observation, or referral by Library staff.Provides short-term case management for individuals who would benefit from case management servicesEvaluates individuals' identified needs through an intake assessment. Needs may include but are not limited to the following services : housing, mental health, primary care, substance abuse, case management, etc.Provides daily consultation to Library staff as needed in regard to issues related to social service needs of patronsProvides consultation and support to Library staff through de-briefing during and / or after an incident with patron(s) has occurredProvides and / or arranges staff training pertaining to topics that might be beneficial in enhancing APL staff's understanding of homelessness, mental illness, and substance abuse.Makes local resource referrals for various services to library patrons tailored to their requests and needs assessment information. Refers patrons who meet service criteria during needs assessment to appropriate service providers, and / or other appropriate agencies based on patron's needsConnects, builds, and maintains collaborative relationships with community agencies and organizations that provide services beneficial to library customers.Performs biopsychosocial assessments, partners with clients to create client centered goals, records all progress notes in chart. Works with other community service providers to enhance the clinical and supportive housing services. Assists with the connection to mainstream resources including healthcare, mental healthcare, benefits, substance use rehabilitation and treatment, etc. Advocates for the needs of individual clients as well as for systemic change, whenever needed, for the overall benefit of homeless individuals. Participates in daily team meetings to coordinate activities with other team members, programs, and the Case Manager Lead.Works to cultivate a culture of support and community within the Amarillo Public Library, Coming Home team and the Community Development department. Communicates clinical assessments, interventions, and ongoing client contact daily, in client record. Works with primary clients, as well as team clients, in service planning, documentation of progress, service tracking, outcome evaluation, and SMART goal setting. Monitors client progress by regular, ongoing contact with individuals receiving services and maintains contact with collaborative service providers, both external and internal.Transports clients using a COA vehicle. Collaborates and coordinates with internal and external programs to facilitate client referrals and to ensure continuity of care; serves as a liaison with collaborating agencies, establishes and maintains effective and professional working relationships with organizations and groups providing services to the target population.Assists others with work that needs to be completed in moments of urgency or crisis.Must carry a company cell phone and be accessible when in the field to the program clients, community partners, APL staff, and Case Manager Lead.Navigates the Social Security, Medicaid, and Medicare systems on behalf of and / or with clients. Completes Medicaid authorizations, HMIS data entry, as well as any other reporting requirements necessary to obtain and sustain funding and provide services for clients. Meets formally with the Case Manager Lead for ongoing individual and group supervision.Attends and participates in clinical and administrative meetings as deemed appropriate and necessary for the job. Assists regularly with client moves, including assembly, pickup, delivery, and set-up of household items. Performs other job- and department-related duties as assigned. MINIMUM REQUIREMENTS A Bachelor's degree in Social Work or a related field is required. A combination of education and field experience may be considered in lieu of a degree. Must have a valid Texas driver's license. PREFERRED REQUIREMENTS A Master's degree in Social Work or a related field is preferred. Two years of experience with homeless population or two years in a related direct human service field working with underserved populations is preferred.KNOWLEDGE, SKILLS, AND ABILITIESMust demonstrate ability to work effectively with a diverse array of co-workers, citizens, investment and property owners, and community groups.Ability to operate small office equipment, including copy machines or multi-line telephone systems.Ability to efficiently operate a computer both in the field and in the office for data entry, email correspondence, and other daily record keeping as required. Must have strong verbal and written communication skills.Must be comfortable working outside of the office, within the community.May require after-hour work to attend meetings, assist clients, finish projects, and meet deadlines.Will be required to drive a City of Amarillo vehicle.Will work Monday-Friday, 8am to 5pm.ADA PROFILEReasonable accommodation will be made to enable individuals with disabilities to perform the essential functions of this position.
