Primary Objective

The City of Bloomington seeks an experienced and dynamic Public Health leader to oversee population health services across Bloomington, Edina, and Richfield. The Population Health Manager will lead a dedicated team to fulfill the agency’s mission to engage the community in promoting, protecting, and improving the health of all residents. This role focuses on leadership, strategic planning, health equity, community partnerships, and measurable program outcomes.

Examples of Duties

Leadership and Program Oversight

  • Plan, organize, and manage the Population Health Team, including the following program areas:
    • Community Resilience Team: Emergency Preparedness and the Statewide Health Improvement Partnership (SHIP)
    • Substance Use Prevention and Policy Team: Opioids, cannabis, tobacco prevention, mental wellbeing, and health-in-all-policies initiatives
    • Community Resource Team: Resource connections, public health nuisance response, public health interventions using evidence-based population best practices and division-wide outreach and engagement
  • Provide leadership, supervision, and mentorship to team members to ensure high-quality, equitable, and person-centered services
  • Develop and implement work plans, policies, and service delivery standards, ensuring alignment with organizational goals and public health priorities
  • Oversee program budgets, grants, contracts, and procurement processes in collaboration with internal and external partners
  • Analyze program goals, data, and outcomes, mobilizing the team to meet performance expectations and measurable results
Assessment and Data-Driven Decision-Making

  • Use surveillance data, best practices, and community input to inform decision-making and design effective interventions
  • Ensure ethical principles are upheld in the collection, maintenance, and use of data, focusing on transparency and impact
  • Continuously monitor program performance, identify emerging trends and recommend data-driven strategies for continuous quality improvement and resource allocation

Health Equity and Policy Development

  • Embed health equity principles into all programs, policies, and decision-making processes to address systemic disparities and social determinants of health
  • Review policies and programs for adverse equity impacts, identifying and implementing improvements to advance inclusive outcomes
  • Support trauma-informed and culturally responsive practices, ensuring services are accessible for all ages, abilities, and communities
  • Evaluate legislative and policy changes, contributing to program development and management team decisions

Community Partnership Development

  • Cultivate and maintain strategic partnerships with community organizations, healthcare systems, and government agencies to align priorities and maximize impact
  • Collaborate with community members, leaders, and coalitions to develop prevention plans and address emerging health issues
  • Serve as a representative of the city, advocating for residents’ needs while fostering cross-departmental and cross-sector coordination

Accountability and Performance Management

  • Establish performance expectations, monitor progress, and hold team members accountable for achieving results
  • Evaluate staff performance, provide feedback, and resolve personnel issues as needed
  • Ensure efficient service delivery and lead quality assurance and continuous improvement initiatives
  • Recommend systems improvements to enhance program operations, workflows, and outcomes

Emergency Preparedness and Response

  • Provide leadership during public health emergencies and support the department’s emergency response plan
  • Fulfill designated roles in response efforts, ensuring preparedness and collaboration across programs

Communication and Reporting

  • Develop and share program reports, assessments, and presentations with stakeholders, leadership, and the community
  • Educate and inform the public and partner organizations about population health programs, priorities, and resources
  • Maintain clear and ongoing communication with internal teams and external partners to align efforts

Knowledge, Skills, and Abilities Required

  • Strong knowledge of public health principles, program development, and health equity frameworks
  • Proficiency in strategic planning, quality improvement, and performance management
  • Expertise in data analysis, health assessment, and using evidence-based practices to inform decisions
  • Proven ability to lead, inspire, and cultivate growth within diverse, cross-functional teams
  • Exceptional project management, organizational, and leadership skills
  • Ability to build trusted partnerships and collaborate with diverse stakeholders
  • Strong problem-solving, critical thinking, and communication skills

Minimum/Desirable Qualifications

Minimum Qualifications

  • Bachelor’s degree in Public Health, Community Health, Public Policy, Project Management, Nursing, or a related field
  • Six (6) years of progressive experience leading public health projects or programs
  • Two (2) years of experience in a supervisory role overseeing direct service staff
  • One (1) year of experience coordinating programs in at least one of the following areas: SHIP, Emergency Preparedness, Health-in-All-Policies, Substance Use Prevention, Mental Wellbeing, Older Adult Health, or Community Health Work
  • Two (2) years of experience with project management, managing grants, budgets, and project deliverables
  • Experience building and maintaining partnerships to achieve shared goals
  • Ability to work flexible hours as needed and access to reliable transportation

Desirable Qualifications

  • Master’s degree in Community Health, Public Policy, Project Management, Nursing, or a related field
  • Experience managing performance measurement systems or leading quality improvement initiatives
  • Experience with developing new programs with grant funding and community-driven outcomes
  • Two (2) or more years of experience managing budgets and staff in a culturally diverse setting
  • Knowledge of city, county, and state public health systems and foundational public health responsibilities
  • Experience with public health electronic health records, data systems, and statistical analysis tools (e.g., R, Tableau, GIS)
  • Ability to speak a second language commonly used in the community (e.g., Spanish, Somali, Vietnamese)

  • Supervise staff within the Population Health Team, including program supervisors, nurses, community health workers, and program specialists

  • The Population Health Program Manager plays a key leadership role in advancing health equity, fostering community partnerships, and improving population health outcomes for residents across Bloomington, Edina, and Richfield

This position supervises three supervisors in the Population Health Team

Anticipated Hiring Range: $101,103 – $103,633 Annually

2025 Full Salary Range: $101,103 – $134,787 Annually
