Job Description
Do you have a passion for helping others? Are you a natural at listening and assisting with mental health matters? If so, come join the Chesapeake Integrated Behavioral Healthcare [CIBH] team as a Clinician II.
Mental Health Case Management Services assist individuals with serious and persistent mental illnesses. Clinicians assist clients with obtaining and maintaining entitlements, linking to needed services, monitoring treatment compliance, coordinating care between service providers, providing supportive counseling, providing education on mental illness and medication, creating service plans, and documenting services provided.
Clinician II (Case manager, Counselor) typical tasks include the following:
Client assessment, treatment planning, and monitoring, including community outreach, help clients maintain treatment compliance.
Documentation in the electronic medical record of all services provided, quarterly reviews of client progress, and of active service plans in a timely manner and in a format to meet Medicaid and DBHDS standards.
Coordination of client services with the client, families, and service providers and advocacy on behalf of clients.
Assist with discharge planning for clients that have been hospitalized including linkage to community resources.
Good communication skills and knowledge of a wide variety of community resources; ability to find new resources that can be accessed by clients.

Required Qualifications
VOCATIONAL/EDUCATIONALREQUIREMENT:
Requires a bachelor’s degree in social work, psychology, closely related field, or another degree as approved by the Virginia Department of Health Professions.
EXPERIENCEREQUIREMENT:
In addition to satisfying the vocational/education standards, this position requires a minimum of two years of related, full-time equivalent experience.
SPECIALCERTIFICATIONSANDLICENSES:
Requires a valid driver’s license and driving record in compliance with City Driving Standards.
Requires Qualified Mental Health Professional (QMHP – Adult or Child)
CPR, First Aid within 3 months of hire
Certified Substance Abuse Counselor (CSAC) certification upon hire
Case Management Modules completed within thirty days of hire.
SPECIALREQUIREMENT:
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.
Preferred Qualifications
Bachelor’s degree and at least three years of experience providing services to mentally ill clients in the social work, counseling, psycho-social rehabilitation or case management fields.
Valid driver’s license with acceptable driving record
