A unique opportunity to work with a dynamic team of public health professionals and community partners in a new and growing home visitation program. Family Connects International (FCI) is an evidence-based model that takes a universal approach offering a free and voluntary nurse home visit to all families of newborns in Lancaster County. Visits include mother and baby nurse assessments, education, and discussion of next steps to address opportunities and concerns including connection to local resources to meet each family's unique needs.
Primary responsibilities include visiting birthing hospitals daily to engage, recruit and schedule home visits; manage nurse schedule and client database; conduct post-visit connection calls to track services received and provide additional referrals as needed; establish relationships with providers and other community partners; maintain local resource guide of community services; market the program to referral sources; and, provide an array of options for pregnant women and new mothers to learn about and sign up for a home visit.
It is beneficial to have experience working with families, providing direct customer service, and delivering presentations to groups.

Qualifications : High school graduate or equivalent and six months of experience with local, state and federal assistance programs including health care and social service systems; or any equivalent combination of training and experience.
Necessary Special Requirement : Possession of a valid driver's license when operating a vehicle is necessary for the satisfactory performance of assigned duties.
