    HEALTH CLUB REIMBURSEMENT

    125.00 / month max for monthly dues

    EMPLOYEE ASSISTANCE PROGRAM

    Three (3) sessions per incident per family member. Unlimited telephonic consultations.

    SEVERANCE PAY

    Three months severance pay in the event EE is terminated for other than cause. Includes EE salary benefits.
