Reporting to the Manager, Central Library & Community Development, the Library Outreach Worker will provide and coordinate services to individuals experiencing homelessness or who are at risk of homelessness. The Library Outreach Worker will support individuals (16 years and older) who are experiencing homelessness, living in the community and accessing Mississauga Library, and will assist individuals in developing a housing plan and connect them with health and social services.
Duties and Responsibilities
System Navigation
- Develop a system of care approach for those who are experiencing homelessness by developing a collaboration and coordination of services with our partner community agencies.
- Conduct basic assessments.
- Provide services and support to youth and adult populations, and make referrals to partnering organizations.
- Provide advocacy where required and assist individuals to self-advocate.
- Develop electronic records, reports and statistics necessary for program functioning.
