Position Overview:
The Community Health Coordinator plays a vital role in improving the overall health and well-being of the New Haven Community by coordinating and implementing programs and services related to both physical and mental health. This position requires working closely with local organizations, healthcare providers, public health and safety officials, community leaders and individuals to address health issues, raise awareness and provide resources that help promote healthier lifestyles and equitable access to care.
Primary Responsibilities:
· Program Development & Coordination: Plan, develop, implement, and evaluate health programs and initiatives tailored to the specific needs of the community.
· Health Education & Promotion: Provide education and outreach to individuals and groups regarding health issues, preventive care, and available health services, including mental health.
· Resource Referral & Advocacy: Assist community members in accessing healthcare services, resources, and support systems; advocate for individuals who face barriers to care.
· Collaboration: Work with local healthcare providers, schools, businesses, and social services to foster partnerships and create a network of support for health initiatives.
· Data Collection & Reporting: Collect and maintain data on community health trends, program outcomes, and resource utilization; prepare reports for stakeholders, funders, and government agencies.
· Community Engagement: Host health workshops, screenings, and events that encourage community participation in health programs; empower community members to take an active role in their health.
· Cultural Competency: Tailor programs and communications to meet the diverse needs of the community, ensuring cultural sensitivity and inclusivity in all activities.
· Grant Writing & Fundraising: Assist in securing funding for health initiatives through grant writing, donations, and community fundraising efforts.
Secondary Responsibilities:
· Assist with Community Development activities including events, ribbon cuttings, and press conferences.
· Provide input into social media posts and press releases and Activate New Haven content.
· Provide input into other public information functions, especially those that pertain to this role.
Job Requirements
- Bachelor’s degree in public health, social work, nursing or related field required. Relevant experience may be considered in lieu of education.
- Experience: 2-3 years of experience in community or public health, or related field required. Experience crisis management preferred.
- Knowledge of local health resources, community organizations and health disparities and trends is strongly preferred.

Working Conditions:
· Full time, with occasional evening and weekend hours
· Travel within the community/region

Level of Supervision and Responsibility
· Reports to the Director of Community Development
· Not responsible for other staff
Difficulty of Work
The role performs duties in a modern office environment or in the field under possible adverse weather conditions. Overall physical demands should be considered minimal.
