The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis CHECW works to increase visibility of the harm perpetuated by centuries of racist socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community faithbased and health care organizations. CHECWs work focuses on social determinants of health including environmental and commercial determinants and addresses both upstream and downstream factors to improve health and well being of New Yorkers. CHECW is comprised of the Bureau of Bronx Neighborhood Health the Bureau of Brooklyn Neighborhood Health the Bureau of Harlem Neighborhood Health the Bureau of Chronic Disease Prevention the Bureau of Health Equity Capacity Building the Bureau of Equitable Health Systems and the Bureau of Finance Administration and Services. The divisions Deputy Commissioner also serves as the Agencys Chief Equity Officer and oversee Race to Justice the Agencys internal reform effort to help our staff learn what they can do to better address racial health gaps and improve health outcomes for all New Yorkers. CHECW sits under the Agencys Chief Medical Officer.

The Bureau of Equitable Health Systems is the healthcare systems bureau of DOHMH situated within the Center for Health Equity & Community Wellness and supporting the strategic plan and vision of the Chief Medical Officer.

Our mission is to apply policy evidence and practical expertise to improve equity in health care delivery at the individual organizational financing and systems levels. The Office of Health Insurance Services (OHIS) provides community facing programs and on the ground programmatic work that maximize health insurance coverageopportunities and increases access to social support services such as assistance with SNAP applications to address food insecurities for all New Yorkers. The Office of Health Insurance Services (OHIS) is currently involved in a few high priority community initiatives and has partnered with the New York City Department of Health and Mental Hygienes Article 28 clinics Early Intervention Program and other agency programs; Mayors Office of Immigrant Affairs Human Resource Administration Department for the Aging the GetCoveredNYC Mayors Public Engagement Unit Health and Hospitals and Communitybased Organizations. To ensure full compliance of these partnerships the Bureau of Equitable Health Systems is seeking to hire an Assistant Site Supervisor to provide operational support to the team of Certified Application Counselors at our Corona Health Center location.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO :

Participate in outreach activities planned internally at the health center site and externally at communitybased organizations to address health insurance disparities within the community.

Educate community residents about the NYS of Health Marketplace insurance products and insurance affordability programs; Aged 65 or older Blind and Disabled (ABD) Medicaid and Medicare Savings program (MSP); applying for Medicare; and other government sponsored programs such as the Supplemental Nutrition Assistance Program (SNAP).

Develop and maintain communitybased partnerships and resources to expand outreach activities.

Provide professional assistance as a New York State Certified Application Counselor and prescreen clients for enrollment into health insurance on the NYS of Health Marketplace Exchange.

Record case manage and track all applications until eligibility / coverage is determined by the NYS of Health Marketplace.

Assist the Site Supervisor to manage the daily administrative operations of the office.

Administer the assignment and processing of referrals received and transmission of referral documentation as needed (e.g. NYC Care PEU Consumer Contact Center DIS TB Unified Electronic Medical Records systems).

Maintain inventory of supplies.

Collect signin sheets and report number of clients to the Health Center Facility Manager in the absence of the Site Supervisor.

PREFERRED SKILLS :

Experience working directly with individuals from diverse racial ethnic and socioeconomic backgrounds.

Excellent oral presentation skills.

Experience working with communitybased organizations.

Excellent computer skills.

Certified Application Counselor experience a plus.

Proficiency in languages other than English a plus.

Qualifications :

1. A baccalaureate degree from an accredited college and two years of experience in community work or community centered activities in an area related to the duties described above; or

2. High school graduation or equivalent and six years of experience in community work or community centered activities in an area related to the duties as described above; or

3. Education and / or experience which is equivalent to 1 or 2 above. However all candidates must have at least one year of experience as described in 1 above.
