The Bureau of Public Health Clinics (BPHC) has a mission to improve the sexual heath of all New Yorkers. To achieve this goal the Bureau provides 1) direct clinical services to people seeking sexual health care and 2) services for sexual partners. The BPHC operates 8 sexual health clinics (SHCs) throughout New York City (NYC) and a telemedicine hotline. SHCs have been at the front line for many activities centered on ending the epidemic of HIV / AIDS in New York City. In recent years the DOHMH has expanded its menu of STI prevention and treatment services in its clinics by including extragenital NAAT screening HSV (herpes simplex virus) testing HPV (human papilloma virus) vaccinations Mycoplasma genitalium testing and rapid trichomonas testing. In addition the SHCs provide a choice of contraception options and medication abortion. The BPHC has also begun providing doxy PEP and is developing a gender affirming care program.

The Bureau is seeking to hire a parttime City Medical Specialist to work in the Sexual Health Clinics (40% of time). An additional 40% of time will be spent on clinical research and evaluation projects overseen by the Bureau of Hepatitis HIV and STIs (BHHS). BHHS mission is to improve the lives of New Yorkers by ending transmission illness stigma and inequities related to viral hepatitis HIV and STIs. BHHS does this through testing initiatives; prevention care and treatment programming; epidemiology and surveillance; training and technical assistance; community engagement; social marketing; policy advocacy; and racial equity and social justice initiatives. The remaining time (20%) will be dedicated to weekly outpatient care at a partnering academic medical center where a period of inpatient service will likely also be required.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO : BPHC :

Acting as content expert for HIV (prevention and treatment) related care

Acting as content expert for STI care particularly syphilis.

Obtaining medical and sexual history

Conducting physical examinations

Ordering relevant stat and microbiologic testing

Making presumptive diagnoses of STIs

Ordering treatment for patients with STIs and HIV.

Providing Post Exposure Prophylaxis (PEP) and PreExposure Prophylaxis (PrEP) for HIV prevention

Providing initial HIV treatment for newly diagnosed patients

Providing Doxy PEP to eligible patients for the prevention of STIs

Providing most forms of contraception

Arranging referrals to clinics for other services as needed

Providing health education to patients

Possibly rotating on the telemedicine service

Working closely with PrEP navigators mental health specialists and HIV case managers to provide comprehensive psychosocial and behavioral care

Adhering to Bureau Standards for productivity.

Performing other duties as assigned by the Assistant Commissioner and the Medical Directors.

BHHS :

Identifying scientific questions of interest for BHHS and BPHC

Working with BPHC and BHHS leadership to design and implement study protocols

Developing research protocols and obtaining necessary approvals e.g. IRB MOU data use agreements

Creating data collection tools and data extraction code

Conducting primary and secondary data analyses using statistical software (e.g. SAS)

Developing manuscripts and presentations

Leading or contributing to grant applications specifically for NIH research funding

Leading a community action group (CAG) focused on optimizing delivery of doxy PEP or syphilis pointofcaretesting services in selected sites which involves recruiting other medical providers and community members for the CAG and facilitating monthly CAG meetings

PREFERRED SKILLS :

Bilingual Spanish / English

Residency training in pediatrics internal medicine family practice gynecology or in preventive medicine

Fellowship training in Infectious Diseases

Knowledge of STI and HIV

Experience with electronic medical records.

Experience with statistical methods and software

Excellent written communication skills

Qualifications :

Possession of a valid license to practice medicine in the State of New York; and either :

a) Valid Board Certification issued by the appropriate American Specialty Board in any specialty area required by the agency; or

b) Current approved application on file for admission to the certifying examination given by the appropriate American Specialty Board in any specialty area required by the agency.

To be assigned to Assignment Level II individuals must have in addition to meeting the minimum qualification requirements for Assignment Level I described above one year of satisfactory experience practicing in the specialty area.

To be assigned to Assignment Level III individuals must possess :

a) A valid license to practice medicine in the State of New York; and

b) A Masters degree in Public Health or equivalent Masters degree including or supplemented by graduate credits in epidemiology and biostatistics. Completion of Epidemic Intelligence Service program of the Center for Disease Control and Prevention may be used to substitute for this Masters degree; and

c) At least two years of satisfactory experience after receipt of the Masters degree or equivalent in the practice of epidemiology. Specialty Board Certification or eligibility may be substituted for one year of this experience.

To be assigned to Assignment Level IV individuals must possess :

a) A valid license to practice medicine in the State of New York; and

b) Completion of an approved residency program in an accredited hospital including three years of experience in either internal medicine or family practice or pediatrics or another related specialty area required by the Department of Health and Mental Hygiene; and

c) Valid board certification or board eligibility issued by the appropriate American Subspecialty Board in Infectious Disease Pulmonary Medicine or a related subspecialty; and

d) Completion of an approved fellowship program in an accredited hospital including two years of experience in either infectious diseases or pulmonary medicine or another related subspecialty required by the Department of Health and Mental Hygiene.
