The Family Partnership Meeting Facilitator plays a crucial role in coordinating and leading state-mandated Family Partnership Meetings (FPMs), which serve as a platform for collaboration between families and service providers. This position is dedicated to actively engaging and supporting families involved in child welfare and adult services systems, ensuring they have a voice in the process and access to necessary resources.
In addition to facilitating FPMs, the facilitator is responsible for contributing to continuous quality improvement initiatives. This includes preparing for Child and Family Services Reviews, a critical process that assesses the effectiveness and quality of services provided to families. The facilitator also serves as a mentor and trainer for new employees and interns, equipping them with the skills and knowledge needed to support families effectively and navigate the complexities of the system. Through these efforts, the Family Partnership Meeting Facilitator helps foster a more inclusive and supportive environment for families in need.

Department Hiring Salary Range: $50,624.00- $58,217.60
Essential functions include but are not limited to:- Manages the daily flow of referrals for Family Partnership Meetings (FPM) that come to the Family Engagement unit and schedules these meetings in a timely manner.
- Coordinates FPMs by reaching out to family members, court representatives, community partners, and agency staff involved in making decisions about the safety, permanency, and well-being of children.
- Participates in service coordination meetings and home visits with agency staff, children, and families to identify potential emotional or physical safety concerns that may affect the quality of the FPM.
- Facilitates structured, solution-focused group discussions that empower children and families to voice their concerns while addressing agency-related issues.
- Documents the action plan from the Family Partnership Meeting in the state's case management system and distributes it to all participants.
- Provides in-service training to staff and the community on best practices for Family Engagement.
- Tracks statistical information related to Family Partnership Meetings
- Collaborates with continuous quality improvement staff to enhance the agency’s child welfare practices.
- Participates in case reviews and other continuous quality improvement initiatives as requested.
Two (2) years experience in social services.
Preferred:
Experience working in state or local government child welfare program.
- Valid Driver's License may be required depending on assignment.
Working Hours: Monday- Friday 8:30am- 5:00pm
Signing Bonus: This position is eligible for a one-time $5000 sign on bonus.
