The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.

Today, the CSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The Norfolk Community Services Board is seeking a Developmental Disability Support Case Manager III to be responsible for assessing and serving individuals seeking services from the Norfolk Community Services Board (CSB).  This position supports a caseload of individuals with intellectual and developmental disabilities to promote independence and community integration while assuring health and safety. Complete annual and ongoing assessment of strengths, needs, preferences, and risks. Link individuals to needed resources and supports, including DD Waiver services. Monitor, at minimum, monthly to determine if there are changes in status and if supports are being delivered as expected. Responsible for coordinating care for the individual between multiple providers. Serve as an advocate for individuals to assure desires and preferences are respected and that all supports align with human rights regulations. Responsible for annual, quarterly, and daily documentation and the processing of DD Waiver services authorizations. 


Departmental Hiring Salary Range:  $46,589- $53,577  
Essential functions include but are not limited to:

  • Completes initial and annual assessment of individuals for planning services and supports. 
  • Develops Individual Support Plan based on assessment information in collaboration with the individual and/or family. 
  • Provides linkage, coordination, and monitoring of services/supports to include Medicaid Waiver Services provided by private providers, other community agencies, natural support systems and other units in the NCSB. 
  • Maintains chart documentation that is in compliance with agency guidelines and state regulations. Ensures that documentation substantiates billing for services. 
  • Conducts face to face visits at a minimum of every 90 days and completes monthly activity related to the individual service plan. 
  • Monitors progress, service delivery, and individual satisfaction by providing monthly service to all individuals on assigned caseload. Provides increased frequency of service whenever clinically indicated. 
  • Identifies and accesses resources to meet identified needs and assist individuals in procuring and maintaining resources and entitlements. 
  • Assists individual/family in obtaining needed services on an emergency basis, i.e. hospitalization, housing, food, medications, etc. 
  • CSB Staff roles may include assisting other units at times to assist them with coverage needs, specialized assistance, or to cross-train. 
Bachelor's degree in a human services or related field.

Two years' experience in the Developmental Disabilities field or co-occurring populations preferably in the case management area.

Additional Preferred Experience:
Three years' increasingly responsible applicable experience in support coordination and/or working with individuals with Intellectual and Developmental Disabilities.
Valid Driver’s License may be required depending on assignment.  

Must be certified (or certifiable) in Basic First Aid and CPR. 

This position is eligible for a one-time $5,000 signing bonus. 

Salary : $46,589 - $76,023
