General Statement of the Job & Essential Functions
GENERAL STATEMENT OF JOB
Under general supervision, this position performs case management, service coordination, screening, assessments and discharge planning for consumers and clients. Reports to Program Administrator (Forensic Services).
ESSENTIAL JOB FUNCTIONS
Prepares forensic case management assessments; provides supportive counseling and intervention; provides consultation to consumers to assess their social, health and vocational needs; assists consumers in obtaining information regarding appropriate community services and resources in the Community Service Board (CSB) of residence; and coordinates goals by linking consumer to appropriate services with the case manager in are residence. Prepares discharge plans for consumers.
Prepares mental health statistical reports and monitors goals and outcomes following established criteria. Presents any trends to supervisor to assist in identifying project outcomes. Maintains accurate and complete clinical charts; prepares and updates consumers' progress records and treatment plans. Keeps required data as requested by the Fiscal Agent and supervisor.
Consults regularly with therapists, staff members, and doctors to discuss consumer progress. Updates consumers' progress records in case file; monitors mental status of consumers throughout programs stay; records observations; and evaluates and monitors progress through service delivery to prepare consumer for discharge planning.
Assists with consumer pre-screening and intake interviews as needed.
Plans and attends workshops, trainings, meetings related to the development of programs to meet consumer needs. Attends case reviews and staff cases and review and monitor recommended interventions and progress. Ensures program policies and procedures are followed.
Performs other related work as required. Individual assignments will be determined by supervisor based on current workload and department needs.

Designs and formulates ways, means and timing to achieve the goals and objectives of the position. Education, Experience & Special Requirements
EDUCATION & EXPERIENCE
Bachelor's degree in Social Work, Psychology, Education or a related human services field and requires 1-3 years related experience working with mental health consumers; or any equivalent combination of training and experience which provides the education and experience. A master's degree is preferred.
SPECIAL REQUIREMENTS
Requires a professional certification in the State of Virginia as a Qualified Mental Health Professional (QMHP) Adult or Child or QMHP eligible.
Must possess Red Cross Cardiopulmonary Resuscitation (CPR) and First Aid certifications or have the ability to obtain certifications within 90 days from date of hire and maintain them as a condition of continued employment.
An acceptable general background check to include a local, state, and federal criminal history check and sex offender registry check.
A valid driver's license with an acceptable driving record.
In the event of a declared emergency in the city of Portsmouth, individuals in this position are required to work shelter duty and may be called on to perform duties as required to provide for the safety and care of the citizens of the community.
