NATURE OF WORK
Serving the public with responsible program support including assisting families, and clerical work for the Head Start Division.
DISTINGUISHING FEATURES
Primary work for this position includes taking applications from prospective families, supporting staff at socialization events such as sponsored community fairs, collaborating with other community and health agencies, providing transportation to families to and from medical and other health related appointments or events using agency vehicle. This position will also perform clearly defined duties related to providing information to the public in person and on the telephone, health data entry and routine data entry, the collection and maintenance of medical records, filing documents, and performing basic math calculations.
ESSENTIAL FUNCTIONS (These essential duties are only illustrative.)
Carries out the best practice for health and safety procedures concerning children, staff, families, and the community at-large. This includes, but is not limited to understanding and implementing the Code of Conduct and active supervision principles.
Supports program operations through primary outreach and communication to parents in the enrollment and administrative process.
Transports families to and from medical and other health related appointments and events in agency vehicle.
Supports early childhood teachers and staff at sponsored events, community fairs, and school family socialization events.
Completes applications with families, with the option of traveling to families’ homes to complete the application forms.
Assists Home Visiting Team with families who have identified specialty health related needs.
Collects and maintains complete and current health and dental records for each child including: vision, hearing, growth assessments, dental exams and treatment, health history, well child checks and immunization records. This may also require travel.
Assists children and families in receiving appropriate health and dental services by communicating with health care organizations and community entities as needed: sets appointments.
Answers, screens, and forwards telephone calls; records and relays messages and appointments as necessary.
Helps current and prospective Head Start/Early Head Start families navigate through appropriate departments, divisions, and employees within organization to ensure their needs are being met.
Enters and extracts records, reports and charts, and performs simple mathematical calculations as needed.
Types and proofreads forms, memos and program documents; enters applicant information into a computerized data base and processes program or service records. Understands timelines of program compliance.
Maintains program correspondence, clerical records and maintains files, supplies inventory, logs, and/or program records. Extracts data and/or files in accordance with established procedures, schedules, or legal requirements.
Copies and distributes files, records, reports and other materials as required.
Maintains attendance in compliance with company policy that also meets requirements to perform duties outlined in job description.
Maintains a safe driving record.
SUPERVISION RECEIVED
Work is performed under the direction of the Health Services Team Leader. Additional guidance is received from established policies, procedures or practices. The employee must be capable of prioritizing a variety of duties and communicate effectively with the public. Work is reviewed through observation of work performance, adherence to policies and procedures and through annual performance appraisals. SUPERVISION EXERCISED

WORKING CONDITIONS & PHYSICAL DEMANDS
Program Support work involves assisting with transportation of families and assisting with socialization activities, with the guidance of home visitor teachers. Some driving of a city-owned or personal vehicle is required. Data entry work is performed primarily in an office setting which may include any of the Head Start/Early Head Start sites. Occasional walking, standing, lifting, bending, or minimal physical activities carrying items under 25 pounds such as books, papers, medical/dental supplies etc. Some tasks have set deadlines and volume processing.

Knowledge of infant and toddler well child check and immunization schedules.
Knowledge of community resources and family health services.
Ability to maintain and update medical records filing system.
Ability to establish and maintain good working relationships with children, families, health professionals, and other collaborators.
Ability to maintain a safe driving record.
Knowledge of modern telephone and office equipment, practices and procedures.
Some knowledge of clerical and secretarial practices and procedures.
Ability to learn City services, departmental operations and organization.
Ability to effectively and efficiently operate a personal computer.
Ability to perform basic mathematical calculations.
Ability to understand and follow departmental policies and procedures, both oral and written.
EDUCATION, TRAINING & EXPERIENCE
Possession of a High School Diploma or G.E.D. and CNA or above.
NECESSARY SPECIAL REQUIREMENTS
Possession of a valid IL Driver’s License.
Required DCFS background check.
Residency Requirement:
Modified November 15, 2021: All employees (except Police Sworn and Fire Sworn) hired on or after June 1, 2018, shall reside anywhere in Winnebago County or within fifteen (15) miles of Rockford City Hall (425 E. State Street Rockford, IL 61104) within six (6) months after their introductory period.
Effective June 1, 2018 all newly hired employees, including newly hired Department Heads, shall live within the municipal boundaries of City of Rockford within six (6) months of their completion of their introductory period.
Classified employees hired after January 1, 1984 may live anywhere in Winnebago County or anywhere within an area fifteen (15) miles from the Public Safety Building within six (6) months of their date of completion of probation. Employees hired prior to January 1, 1984, shall be subject to their conditions of employment in effect at that time.
