Aging and Disability Services division (ADS) manages the continuum of care that supports older individuals and those living with a disability, and their families. The mission of Aging and Disability Services is to develop a community that promotes quality of life, independence and choice for older people and adults with disabilities in King County.  Our goal is to promote healthy aging, ensuring that our communities are great places for people to grow up and grow old.  

 

As the state-designated local Area Agency on Aging (AAA) for Seattle and King County, ADS work in partnership with King County to: 

• Improve the health and quality of life for older adults and adults with disabilities
• Connect seniors and adults with disabilities with helpful resources.
• Provide help and support for caregivers.

The City of Seattle Human Services Department is seeking Registered Nurse Consultant to join the Aging & Disability Services Division.   The successful candidate will join a team of enthusiastic, energetic, and dedicated colleagues who provide home-based case management services to vulnerable adults throughout King County.    The RN Consultant helps support individuals with complex medical conditions and will collaborate with medical providers and other key partners to maximize health care resources and positive health outcomes for clients.

 

The successful candidate will have the skills and compassion to work with a diverse group of case managers and clients, including working with clients who have difficulty sustaining basic living needs due to their physical, emotional, behavioral or cognitive status, and physical or emotional neglect or abuse. Additionally, many clients experience drug and alcohol issues, and require compassion, advocacy, support, and connection to resources and services. We're looking for a highly professional RN Consultant who is also comfortable working with ethnically diverse adults who are eligible for Medicaid in home personal care.  

The RN Consultant will be a key member of the client’s healthcare team, providing consultation to Case Managers in the Aging and Disability Services Case Management Program.  RN Consultants with ADS do not provide hands-on clinical care to clients but do make home visits to provide teaching and education to clients and caregivers, they observe/assess for any skin breakdown, verify treatment plans and make appropriate referrals.  

Client population includes ethnically diverse older individuals and adults with disabilities, living at home, and are eligible for Medicaid in-home person care services and supports. These individuals may have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol issues. These positions report to the Nursing Services Manager.

 

The City of Seattle provides a great benefit package, including reimbursement for RN licensure fees and a budget for continuing education.  Working for the city makes you eligible to apply for the Public Service Loan Forgiveness Program. While the exact schedule may vary, this position is generally expected to work a standard Monday – Friday schedule during regular business hours and required to be in the office/field a minimum of three days a week.

 

Position is based out of the Seattle Municipal Tower (700 5^th Ave, Seattle) and will provide services throughout King County, including South King County and support our subcontracted providers as necessary.   

Registered Nurse Consultants provide medical expertise, input and consultation to a team of social work case managers whose clients are at high risk of residential placement or hospitalization due to unstable health conditions or caregiver/provider situations.  Client population consists of frail, elderly, disabled adults receiving Medicaid funded services with multiple risk factors including physical and/or mental health problems, long-term care needs, drug/alcohol issues, behavioral or cognitive status, and physical or emotional neglect or abuse.

 

• Work closely with case managers to develop a plan of care.
• Coordinate with other providers such as primary care, specialty, mental health and home health.
• Meet with clients in community or their homes to assess their needs.
• Conduct skin observation for pressure injuries and other wounds and make recommendations for care.
• Provide education and training to caregivers, clients and staff.
• Make referrals to other services such as Adult Day Health and medication management.
• Use Care Transitions strategies to help clients avoid unnecessary re-hospitalization.
• Document findings into CARE on a assigned laptop during file review and nursing interventions.
• Use your keen critical thinking skills to identify gaps in care plans and help case managers to fill those gaps.
• You may be asked to carry a small caseload of clinically complex clients.
• Find opportunities to advocate for clients and dismantle barriers. 

Minimum Qualifications:  

 

• A Bachelor of Science in Nursing
• 2 years of clinical nursing experience with elderly and/or disabled adults OR
• 3 years of clinical nursing experience with any age group

      OR: Or a combination of education, training, and/or work experience which provides the ability to perform the work of the position.

License, Certification & Other Requirements

• Washington State Registered Nurse License in current, good standing
• Current BLS Certification
• Current State of Washington Driver’s License or equivalent mobility, with reliable personal transportation
• Ability to lift, pull or push at least 25 pounds.
• Current and complete vaccination or occupational health record or proof of immunity.

Desired Qualifications:  

• 2 years of home visits in a clinical nursing experience in a home health or hospice 
• Second language fluence in Spanish, Russian/Ukraine, Somali, Punjabi and/or Bosnian with ability to effectively communicate orally and in writing in English and second language is preferred.

Technical Skills

• Prior experience using electronic medical records to record client information and ability to learn new systems.
• Ability to navigate multiple screens for case review
• Prior experience using and understanding Medicaid systems
• Prior experience and proficiency using standard MS Office software such as Outlook, SharePoint, Word and Excel
• Prior experience with skill and experience in using computerized CARE assessment program is highly preferred.

Clinical Skills

• Strong clinical assessment, planning and comprehensive, narrative documentation skills such as DAP or SOAP notes
• Extensive experience with chronic illnesses/co-morbidities in a variety of settings, including residential and home health care as well as family practices and clinics.
• Experience using Motivational Interviewing to identify clients’ needs and challenges as well as assist clients with behavior change.
• Experience with Care Transitions strategies.
• Ability to coordinate care plans with social services, physicians, home health agencies, hospitals, and health industry providers.
• Experience working with elderly or disabled adults who have unstable health conditions and risk factors including multiple physical and/or mental health problems, long-term care needs, and/or drug/alcohol problems. 
• Ability to stage a wound and make appropriate treatment recommendations.
• Experience in case management of clinically complex clients.
