Position Purpose / Summary
Responsible for managing SHINE outreach duties and activities by assisting Medicare beneficiaries in attaining affordable health insurance within Hampden and Hampshire Counties. Delivers presentations to underserved populations, hosts health fairs and exhibits, and meets with elder residents to assist with applying for products to meet their health care needs. Work is performed under the supervision of the Director of Elder Affairs. Technical guidance may be exercised over SHINE personnel.
Essential Functions
- Assist Medicare beneficiaries by assessing health insurance gaps, filling out necessary applications and working to attain necessary insurance coverage in person, by phone or video. Works with the beneficiary and their support system to sort out paperwork, billing statements, and provider information to ensure the elder has complete coverage.
- Assist with managing the SHINE office personnel, including office Program volunteers.
- As assigned by SHINE Coordinator, coordinate social media and print publicity, marketing and outreach to alert consumers of the program's mission and availability.
- Assists with coordinating outreach events and presentations to underserved populations.
- Prepare SHINE information display materials for outreach events and presentations.
- Host educational events for community advocates, outreach workers, social workers and others serving the Medicare population.
- Assist with supporting volunteer counselors and respond to questions, requests for technical assistance, or advice in handling cases.
- Help to develop relationships with new host agencies for new SHINE counseling sites.
- Assist the SHINE Coordinator in training volunteers and matching volunteers to Community site assignments.
- Assist the SHINE Coordinator with assessing counselor competence and performance through site visits, role playing, customer satisfaction surveys and case reviews.
- Attend monthly State SHINE Regional Directors meetings to maintain State Certification and SHINE Grant compliance; in-state travel required. Attend other monthly specialized National and State training sessions as assigned by SHINE Regional Director.
- Attend conferences and other meetings relevant to SHINE program goals to maintain State Certification and SHINE Grant compliance as assigned by SHINE Regional Director.
- Respond to voice mail and email, taking appropriate action (i.e. refer to Regional SHINE Director, return calls, refer to other counselors, etc.) Refer appropriate clients to Professional Community partners including Dept. of Elder Affairs to meet other client needs.
- Communicate with the Regional SHINE Director on problems arising ( i.e. network, clients, referrals)
- Performs other related duties as required.
