A $3,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.
- $750 will be awarded in the first paycheck.
- $1,000 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.
- $1,250 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.
The City of Virginia Beach Department of Human Services is seeking to hire a Licensed Practical Nurses (LPN) who has nursing experience working with adults with mental illness, substance use, and or co-occurring disorders to provide primary care screening services to patients in the Adult Outpatient Services Program in the Behavioral Health Division.
Duties for the first Licensed Practical Nurse(LPN) position:
- Monitor and screen all clients on anti-psychotic medications for Metabolic Syndrome to include completing extensive documentation in both the paper record and electronic health record (primary care screening) (blood pressure, pulse, weight, BMI, A1C, Glucose Monitoring and Cholesterol Levels).
- Preform venipuncture, prepare laboratory requisition forms when blood work is ordered and ensures laboratory test results are in the medical record.
- Track laboratory results but not limited to, vital signs, transcription of physician orders and consultation with Prescriber regarding PCP follow up where applicable.
- Collaborate with internal and external care providers to provide the highest level of safety, efficient and effective service delivery.
- Collaborate with prescribers in regards to lab orders and abnormal values.
- Communicate with outside providers for follow up care for patients; Maintain communication with clients and their families regarding the importance of follow up care with outside prescribers
- Collaborate with primary care providers for continuity of care and link clients without a primary care provider to a resource to obtain medical care.
- Address barriers to treatment follow up with clients and families.
- Provides plan for follow up with clients who no show to PCP appointments and works with clients to keep medical appointments.
- Maintain clinical records in accordance with regulatory standards, including Department of Human Services, DMAS, and licensure; obtains client/guardian signatures on required forms. This includes entering progress notes in the EHR for all client interactions including, but not limited to, scheduled appointments, injections administered, calls and correspondence with clients.
