BRIEF DESCRIPTION:
The purpose of this position is to provide intake and support services to clinic personnel. This is accomplished by completing eligibility and registration forms, answering phone and walk-in inquiries, creating and maintaining charts and files, completing initial assessments and screenings, and ensuring medical and education information is readily available. Other duties include calculating payments, providing support to the nursing staff, assisting with home visits, making appointments, and completing general administrative tasks.

1 L Regular and reliable attendance in compliance with given schedules is a mandatory requirement of this position. Schedules may be subject to change with or without advance notice
2 S Assists with clinic registrations by reviewing applications and assisting with eligibility determination, making and changing appointments, creating, pulling and updating charts and files, determining payments, processing requests from funding sources, and ensuring supplies and equipment are stocked.
3 S Supports clinic activities by entering chart information, building client files, explaining paperwork and clinic policies, obtaining signatures for tests, printing labels and lab slips, assisting nurses with immunizations, lab draws and tests, taking vital signs and screening information, and completing simple tests.
4 S Maintains supply inventories by documenting medications dispensed, documenting tests performed, reconciling inventories, restocking, and ordering supplies
5 S Maintains files, databases and other information on clients and services by updating charts and files, and documenting all contacts.
6 S Completes administrative tasks by answering the phone and processing inquiries, answering program questions, providing eligibility paperwork, and processing mail.
