Job Description:
The Member Navigator is accountable for helping low complexity members navigate and coordinate health needs and social resources.
Responsibilities:
Cares for members, identifies and addresses barriers to and social influences on optimal health.
Coordinates care for a case load of members, identifying and addressing their barriers to and social influences on good health.
Conduct assessments and screening instruments, including those for behavioral health, home safety, and social needs, while adhering to established protocols and maintaining regular check-ins with members.
Work Experience:
2 years of progressive experience in care coordination, community health, health promotion, or a related field.
Education:
Bachelor's degree

The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:
Min: $26.46
MRP: $29.40
Max: $32.34
