JOB SUMMARY
The San Jose Renew Aftercare Coordinator will extend the case management of Renew men and women graduates for one to two years. The Manager will work with graduates individually and in groups. Renew Case Managers, Forward Housing House Managers, and Intern Managers may provide direct case management of graduates but will coordinate their efforts with the Aftercare Coordinator. The Aftercare Coordinator must demonstrate high relational and motivational skills along with strong administrative and computer skills. This position will require regular evening and weekend work to facilitate group events and meetings with individuals after work.
Job Duties
- Develop and implement an aftercare strategic plan that provides aftercare for all San Jose Renew Graduates for a minimum of one year after graduation
- Work closely with Renew and @Work staff in coordinating the aftercare strategic plan and collecting post-graduates’ data
- Case manages monthly meeting with Graduates to support and encourage graduates in their sobriety and assist them with employment and other needs that arise
- Network with churches and other organizations to provide resources and other social groups for our graduates
- Establish volunteer opportunities with CityTeam programs and support services
- Develop and offer counseling/mental health resources
- Develop and offer family support events for graduates
- Supervise Forward program house meetings and AA meetings

Requirements
Physical Demands
- Stand and sit for prolonged amounts of time

- Frequently lift and move up to 20 pounds
- Finger and hand dexterity to feel, handle, and operate objects, tools or controls, and to reach with hands and arms
- Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus
Required Education and Experience
- B.A. or equivalent experience with specific training in Bible, counseling, urban missions, or theology
- Must have a compatible theology on major issues of CityTeam’s Statement of faith
- Must be committed to serving God and the Body of Christ.
- Must have a teachable spirit and committed to team participation.
- Minimum 5 years’ experience in case management field or equivalent
- Minimum 2 years’ experience working with children or education in the field of early childhood development preferred.
- Familiarity with Cityteam, DMM, and addiction recovery principles
- Good interpersonal skills, with proven ability to communicate with people from diverse backgrounds
- Proven skills in motivating people, setting goals, and commitment to getting the job done
- Must have a valid California driver’s license with no more than two-points and no DUI related incidents in the past ten years
Preferred Education and Experience
- CAADAC/CCAPP certification
- Prefer a graduate of a CityTeam Renew program
- Bilingual language skills (English/Spanish/Vietnamese/Cambodian)
- Leading interactive groups & Parenting Class
- Experience and specific course work in the area of chemical addiction, codependency, crisis intervention, and family dynamics
- Good written communication skills, experience writing court reports and or assessments
- Computer literate and proficiency with Microsoft Office Suite
- Proven relational skills in motivating and encouraging people
Work Conditions
- This job operates in a professional office environment and residential apartment setting
- This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
- Must be available to work evenings and weekends
- Valid driver’s license
- Travel is primarily local during the business day

- Must have an intimate, personal relationship with Jesus Christ
- Must be active in a local Christian body of believers or at least pursuing community with other Christ-followers
- Must have compatible theology on major issues of CityTeam's Statement of Faith
