About This Position:
While the Clinicians in the Community Diversion program (16-bed program) will focus on assessment and treatment targets, the Case Managers will collaborate with the individual and family to identify the outstanding educational, vocational, and aftercare needs required to support a successful re-entry to the youth’s home environment upon discharge. The first point of contact with the child and parent/guardian will be with the Case Manager at screening, and eventual admission. Any planning, support, or advocacy needed around these areas will be facilitated by the Case Manager. The Case Managers will also co-facilitate DBT practices in the milieu including mindfulness and skills groups, and support milieu staff with skill acquisition as needed.

- At assignment of referral- the Case Manager will schedule and complete the screening, admission, and intake to the 16-bed program
- In the event, there are concerns raised in the screening process, the Case Manager will share this information with the Clinical Program Director to initiate a meeting with Probation and JBCSSD Program Monitor
- The Clinical Program Director will manage the assignment of cases to the CM. While maintaining a balance is the goal- the case load will vary based on client and family need, language considerations, and case manager availability
- Become trained in DBT Lite and support the program milieu with achieving DBT proficiency around skill use and recognition as well as coaching of youth
- Provides case management and re-entry/transition planning connecting youth to community resources, employment in their local community, and support re-connecting youth to their community school
- Participate in weekly MDT team meeting and provide updates on re-entry
- Input all case management documentation into the ECR as well data into CDCS within timelines outlined by CSSD
- preparedness, family needs, community challenges, educational/vocational needs, aftercare referrals
- Caseload will vary based on program needs and if vacancies arise as all youth will need to be serviced
- Work with adolescents to develop a resume, obtain a State of Connecticut identification card and support vocational exploration programs
- When a youth is experiencing challenges in the program, that warrant meetings with the case manager will participate in said meetings as available
- In the event aftercare services are delayed in starting, the Case Manager will support the adolescents in the community for up to sixty (60) days to ensure that the adolescent is connected to a school, prosocial activities, and employment as appropriate
- Transport adolescents to appointments, for community reintegration efforts and to facilitate family visitation/sessions and home passes as available
- Facilitates and co-facilitate groups as needed (Restorative Justice, Harm Circles, Re-Entry Circles, Mindfulness Groups, Life Skills Groups, etc.)
- Associate’s Degree required, Bachelor’s degree preferred
- Minimum 3-years’ experience in residential program, 5 years’ preferred
- Lived experience and experience, training, and a history of working with adolescents – Preferred
- Valid driver’s license to utilize company vehicle -Required
- Sunday-Wednesday 7:00am-5:00pm
- $19.81 per hour
- 40 hours per week
- Company vehicle will be provided
