What are the responsibilities and job description for the CLINIC PROJECT NURSE position at CLARA BARTON HOSPITAL?
PURPOSE
Clinic Project nurse will spearhead 'Patient Centered Medical Home' (PCMH) initiatives and work with Physicians and Mid-level providers to help guide patient healthcare. The project nurses are responsible for developing and maintaining the care management program.

The Project nurse reports to Physicians, Mid-levels, Nurse Manager, Clinic Practice Administrator and Chief Nursing Officer

FUNCTIONS
- Perform a variety of patient chart reviews and audits related to PCMH to determine appropriate healthcare needs and improvements.
- May assist the Physician and/or Mid-level practitioner with patients. Prepare patients and examination rooms, take initial medical history for the chart, take vital signs, make lab and x-ray appointments, and explain the preparations to the
- May assist in notifying patients of lab results as requested by Physician and/or Mid-level practitioner.
- Receive and handle patient and other phone calls as directed by the Physician and/or Mid-level practitioner.
- Assist in Health Fair labs by adhocing the labs.
- Will work with outside facilities to receive medical records of ER discharges and Hospital discharges. Will send message to provider nurses when receiving ER discharges and hospital discharges.
- Will work with insurance companies to help notify patients of due or overdue testing or office visits.
- Help patients to obtain patient assistance to help with healthcare costs.
- Complete Medication Prior Authorizations.
- To work at developing and maintaining care management patients and care management process.
- To schedule and plan a monthly care management meeting.
- Assist in making sure the drug sample closet is checked monthly for expired medications and that they are clean and orderly.
- Will be the backup for the receptionists on incoming phone calls
