JOB SUMMARY: The Diabetes Educator/Instructor is a healthcare professional certified as a diabetes educator. The staff member in this position provides individualized diabetes self-management education/training and support (DSMES) to individual patients according to Scope of Practice, Standards of Practice, and standards of Professional Performance for Diabetes Educators and AADE accreditation requirements.
ESSENTIAL FUNCTIONS
Instruction of program participants
Clinical & Depart Duties:
- Collects DSMES program participant’s assessment data and completes individual and comprehensive patient assessment on each participant.
- Develops individualized educational plan with participant that includes education goals, intervention content, teaching methods and patient-selected desired outcomes.
- Provides DSMES interventions that utilize primarily interactive, collaborative, skill-based training methods and uses the patient empowerment/motivational interviewing style of counseling.
- Develops individualized follow-up DSMES plan with participant.
- Evaluates effectiveness of educational services provided by measuring attainment of agreed upon goals/strategies.
- May participate in development of DSMES program curriculum, patient training materials and hard-copy and/or e-documents that support each facet of DSMES program.
- Contributes to and participates in continuous quality improvement process to measure attainment of DSMES program and to identify and address opportunities for improvement.
- May contribute to and participate in ongoing provider marketing activities in order to maximize DSMES referrals and to ensure existing providers continue to refer.
