Role Description
Sodexo is recruiting for a per diem role for an outpatient Registered Dietitian and Certified Diabetes Care and Education Specialist (CDCES). This is an on site position with approximately 4 hours per week in the evening or on Saturday morning.
Key responsibilities include:
- Ensure Diabetes Self-Management Education and Support (DSMES) program accreditation requirements are met and maintained.
- Provide DSMES to program participants and adhere to national standards for DSMES documentation in electronic medical record.
- Collect DSMES assessment data and outcome measures.
- Perform limited physical exam, glucometer, CGM and insulin teaching as needed.
- Identify needs of patient and deliver care that is specific to those needs, adjusting for culture, literacy, hearing or visually impaired, etc.
Qualifications
- Active Registered Dietitian (RDN) credential
- Active Certified Diabetes Care and Education Specialist (CDE/CDCES) credential
- At least one year of outpatient diabetes experience
- Experience with insulin pumps and continous glucose monitoring technology preferred
- Strong communication and interpersonal skills
- Ablility to work independently and manage time efficiently
