JOB SUMMARY
Community support staff focused on the Emerging Adult and Adolescent populations. As a part of the System of Care grant from DBHDD will provide services that consist of rehabilitative skills building, the development of environmental supports and resources coordination considered essential to assist a person in improving functioning, gaining access to necessary services in creating environments that promote recovery and support the emotional and functional improvement of the individual. The CPS-Y will also participate in the engagement and coordination of care of children, adolescents, and young adults with other systems and internal providers.
DUTIES AND RESPONSIBILITIES
Staff will serve as the primary coordinator of behavioral health services and will provide linkage to the community; general entitlements and psychiatric, substance use/abuse, medical services, crisis prevention and interventions services
Staff will have contact with the individuals on his/her caseload weekly
Staff will meet with consumers face to face a minimum of twice a month with at least 50% of contacts being face to face and 80% of contacts outside of the clinic
SKILL, COMPENTENCY AND EDUCATION AND/OR LICENSURE REQUIREMNTS
Certified Peer Specialist-Youth in Georgia. Must have a current driver’s license
INTER- RELATIONSHIPS
Able to work closely with other staff within the center, including clinicians, prescribers, and front desk staff, as well as other community providers for client referrals and linkage
WORKING CONDITIONS
Community and office setting Monday-Friday (three days per week 8:15am-5:00pm; two days per week 10:15am-7:00pm)
May require weekend or adjusted hours due to program and community events
Ability to use a computer, telephone, fax, copy and scanning machine
Completes/passes agency-sponsored defensive driving course, as well as all other agency-required trainings
Must be able to drive, climb stairs and walk at least 1 block. Services will be provided in home or in the community to include public buildings such as the library and government agencies
Hours of availability can vary based on need of clients to include late evenings and weekends
