Our Pharmacy Liaison plays a vital role in expanding our growing patient base by educating the community, patients, and caregivers about CareFil Rx—one of our premier medication management solutions—and enrolling them in our patient-centered program. Beyond community outreach, the Pharmacy Liaison actively participates in events to highlight the holistic benefits of our MTM solution. They also play a key role in onboarding new patients, gathering essential information to support the admission process. Additionally, they collaborate closely with prescribers and pharmacists to ensure a seamless transition into the CareFil Rx packaging program.
Location: Cincinnati, OH / Dayton, OH
Salary : $20 (with performance bonus based on new patient growth)
Travel: This role combines remote work with in-field presence, requiring regular travel to engage with patients, caregivers, healthcare providers, and community events based on business needs and assigned territory

- Participate in community events and health fairs to educate the public on CareFil Rx and its medication management solutions, promoting awareness of our products, services, and benefits for improved patient outcomes
- Build and maintain trusting consultative relationships with community organizations and healthcare providers managing populations of highly complex, non-adherent patients within an assigned geographic territory
- Engage patients referred to Clarest Health Pharmacy to schedule an admission interview and start the onboarding process
- Conduct admission interviews for patients who are interested in Care F il Rx and collaborate with pharmacy associates to ensure the smooth transition of admitted patients
- Identify barriers to adherence and collaborate with the internal pharmacy team to address and resolve them during the clinical review process
- Provide updates to referral sources on patient status with the pharmacy and address any obstacles
- Support medication synchronization by working with prescribers and transferring pharmacies to obtain needed prescriptions
- Coordinate with patients and caregivers as needed to ensure a seamless pharmacy transition
- The ability to adhere to Clarest’s Code of Conduct, follow Clarest Compliance policies and procedures, and report any suspected violations of any federal or state laws to either their direct supervisor, Human Resources or the Compliance Officer
- Other duties as assigned by the manager to support departmental needs
Qualifications
- Minimum High School Diploma
- Pharmacy Technician experience preferred
- Minimum of 3 years of experience in a healthcare or community-based role, with a focus on patient support and medication management
- Understanding of medication labeling, billing and adjudication
- Valid driver’s license (in good standing)
- Excellent communication (both oral and written), interpersonal, and relationshipbuilding skills to connect with a wide variety of clients i.e., vendors, community, patients, caregivers, and all levels of associates within our pharmacies
- Consultative approach to working with clients, and an ability to identify and solve problems within the scope of responsibilities
- Compassionate and patient-centered approach to care
- Ability to work independently and manage multiple responsibilities and priorities simultaneously and to conclusion
- Lifelong learner who seeks to understand the business and develop strong business acumen Attention to detail
- Proficiency in Microsoft Office Suite and electronic health records (EHR) systems
- Ability to travel within assigned territory
