Home Health Social Worker

CFVNA is seeking a full-time Social Worker (MSW) to join our Home Health Team. The MSW best fit for this position is well organized and has excellent documentation and time management skills.

As a Medical Social Worker, this candidate will play a vital role in helping patients manage the emotional, social, and medical challenges of illness or injury; navigate the healthcare system; and access community resources to meet the patient’s needs.

Responsibilities include but are not limited to:

• Assists the patient and family with health related financial, social, and emotional concerns and assists with effective coping skills
• Provides social work services in accordance with the plan of care
• Ensures that changes in the patient’s clinical status are communicated to other healthcare team members in a timely manner
• Educates the patient, family, and other team members on medical social work interventions and treatment plans
• Performs care planning, transfer, and discharge functions
• Identifies risks or threats to the patient’s health in the patient’s environment and promptly implements appropriate interventions to reduce risks.
• Communicates identification of risks/threats to the patient to the physician and other members of the healthcare team to coordinate care effectively
• Marketing duties as assigned by the agency
• Provides back-up coverage for the Hospice Social Worker and Spiritual and Bereavement Counselor as needed
• ODP Certified Investigator Training Program
• Other duties as assigned

Requirements:

• Bachelor’s degree or Master of Social Work from an accredited program, Master of Social Work preferred
• Minimum 1 year of social work experience in Healthcare setting
• FBI, Child and Criminal Clearances required
• CPR certification required
• Excellent verbal communication skills
• Strong assessment and problem solving skills
• Knowledge of community resources and referral networks
• Must have a current valid PA Driver’s License, reliable transportation, auto insurance
• Able to work in a variety of settings and conditions
• Hours are primarily Monday-Friday 8:00AM-4:00PM.

Hourly wage commensurate with education and experience.
