Job Description:
- Conduct thorough assessments and evaluations of patients with cognitive and/or ADL physical functioning disorders
- Develop and implement effective plans of care based on patient needs and goals
- Collaborate with other healthcare professionals to provide coordinated care and services
- Evaluate treatment outcomes and adjust services as needed
