Description
The Home Care Support Coordinator will serve Home Care and Community Supported Living Members that are frail elders, persons with physical disabilities and/or persons with intellectual/developmental disabilities. The goal for you is to ensure that these Clients receive the services that are authorized for them, which promote their overall well-being. The Home Care Support Coordinator also manages the day-to-day communication flow throughout the office. They carry out responsibilities in the following functional areas: telecommunication, data entry, customer service, departmental support, and documentation/form management.
Essential Responsibilities and Desired Outcomes
- Answer telephones and text messages with professionalism and respond appropriately to the needs of our internal and external customers. Transfer calls, distribute information and take messages as necessary, to ensure timely and effective communication.
- Ensure a welcomed feeling by greeting and assisting visitors/applicants as they arrive at the office.
- Maintain, prepare, and distribute mail & interoffice correspondence daily in order to have efficient flow of information.
- Oversight of the Family & Friends clients and staff to include scheduling, time management, and annual reviews of Family Care providers.
- Ensures proper doctor orders are faxed, completed, and follow up is accurately maintained.
- Requests all doctor notes on new clients for RN prior to admission.
- Completes intake referrals for the Home Care division and checks eligibility.
- Completes all assigned paperwork, documentation and authorizations in a correct and timely fashion.
- Ensures quality and safe delivery of services.
- Proctors Home Care 101 and Sandata training as needed.
- Enter new Home Care clients and staff into Electronic Charting System, i.e. Sandata.
- Maintain current employee’s personnel and demographic information in Electronic Charting System, i.e. Sandata.
- In conjunction & in partnership with scheduling department, assist with maintaining & updating employee exceptions.
- Prepare admission folders for Home Care, Supportive Home Care and Community Supported Living Programs.
- Mail schedules for Home Care, Supportive Home Care and Community Supported Living Programs.
- Prepare and monitor RN Schedules while maintaining the requirements of the payer.
- Contacts clients to set up RN assessments.
- Order office supplies for Green Bay Office, Occupational Health, and Home Care.
- Maintain and monitor all grant programs related to the Home Care division.
- Manage and review Member, petty cash and gas expenditures.
- Cross-sell agency services to all referrals as appropriate to increase Member base and company profits.
- Ensure Member files are accurately and timely maintained.
- Maintain office security by following safety procedures and controlling access via Reception desk.
- Assist with setting up virtual trainings for CCTI and the coinciding documentation.
- Assist the Sunburst Committee with office events.
- Maintains a good working relationship with internal and external customers, to include Care Teams.
