Shelter Case Manager

CCHC is seeking a passionate Shelter Case Manager to join our team and make a real difference in the lives of homeless families and individuals.

In this full-time position, you will report directly to the Executive Director and play a vital role in delivering services and programs that help our clients achieve housing stability. You will be a champion for our clients, supporting them on their journey to self-sufficiency.

What You'll Do:

• Provide housing-focused case management to families and individuals in our transitional housing and emergency shelter programs.
• Collaborate with clients to set goals and track their progress.
• Empower clients by assisting them with tasks like job and housing searches, applications, budgeting, advocacy, and obtaining documentation.
• Meet clients' basic needs by providing resources or referrals for hygiene items, clothing, medical care, and more.
• Connect clients with community resources and partners to equip them with the skills needed for sustainable housing.
• Maintain accurate and detailed client files.
• Represent CCHC at community outreach events and meetings.
• Collaborate effectively with the Housing Manager and other staff members.
• Travel is required for client meetings, community events, trainings, etc.

Who You Are:

• You hold a Bachelor's degree in Social Work or a related human services field (preferred) with a minimum of two years of experience.
• You possess excellent communication, organization, and computer skills, particularly in Word, Excel, KYHMIS, and email.
• You are a fast learner and can become proficient in the KYHMIS data system within three weeks.
• You have a valid driver's license, a reliable vehicle, and valid car insurance.
• You are physically able to climb stairs and lift at least 25 pounds.
• You are self-directed, a problem-solver, and a team player with a pleasant personality and a desire to help those in crisis.
• You are committed to fair housing practices and maintaining a positive and supportive work environment.
• You are able to pass a background check and drug screening.
