What are the responsibilities and job description for the Adult Day Care Aide/Outreach Support Worker position at Clark County Rehabilitation and Living Center?
Our Adult Day Care/Outreach support workers provide supervision, care and support to adults with disabilities. Clark County Rehab & Living Center is seeking a dedicated and caring individual to replace a long term employee retiree.
REQUIREMENTS:
Valid driver’s license and safe driving record
CNA Certification
Demonstrated acceptable work attendance
MUST DEMONSTRATE WITHIN THE 30-DAY QUALIFYING PERIOD THE ABILITY TO MEET ALL WORK EXPECTATIONS INCLUDING THOSE DEFINED BELOW:
1. Must have the ability to demonstrate and conduct formalized programs in the areas of prevocational services, leisure activities, community access, hygiene and personal cares, walking and range of motion exercises, behavioral support, and all areas of independent living skills.
2. Must have the ability to assist in the development of an individual service plan as well as the ability to implement and collect data for all plans.
3. Must be able to work independently with the program participants both in the facility and in the community while supporting them in their formal programming and goals.
4. Must work as an active member of the treatment team in generating ideas and discussion regarding participants to provide a high quality of services for each individual.
5. Must have a pleasant personality, positive attitude, and the ability to work well with coworkers as well as with the participants who attend services. This includes participants with geriatric, mental, physical, and developmental disabilities.
6. Must be able to assist participants with meals, snacks, and drinks per individual plans.
7. Must be able to assist participants with all personal cares per individual plans.
8. Must be able to communicate professionally with case managers, guardians, and other treatment team members both on the phone and in person.
9. Must have flexibility in scheduling as shifts can vary from day to day and could be adjusted on short notice due to census or staffing changes.
10. Must have a valid driver’s license, a reliable vehicle with 4 doors that can transport 4 or more participants safely and comfortably as needed, and must have and submit proof of active and sufficient vehicle insurance. Must also be aware that Driving Record history is subject to review at any time.
11. Must be able to drive county vehicles safely while transporting clients. Must be able to use adaptive tie-downs and seatbelts correctly.
12. Must be currently licensed as a CNA on the Wisconsin Nursing Assistant Registry. Must be qualified to work in a federally certified facility. Must keep license up to date, including completing any shift requirements as directed by the State of Wisconsin.
13. Must complete Medication Administration Training and properly administer medications to participants per doctor’s orders.
14. Must complete First Aid and CPR training and upkeep certification bi-annually.
15. Must be able to lift, carry, push, and/or pull up to 10 pounds continuously and up to 50 pounds frequently. Must also be able to push and/or pull up to 100 pounds frequently.
16. Must be able to work outdoors on a daily basis.

Pay: $17.25 - $21.87 per hour
