Overview of the Role:
The Community Health Improvement Specialist will collaborate with various departments to promote health education within Sanders County. The role aims to improve the health and well-being of individuals and communities by encouraging positive behavior changes, preventing disease, and promoting health. This will be achieved through education, strategy development, and program implementation.
Key Responsibilities:
- Needs Assessment: Identifying health needs of individuals, employees, and the community.
- Planning & Implementation: Developing and executing health education programs and interventions.
- Collaboration: Working with department managers and stakeholders to administer health programs.
- Health Advocacy: Promoting health education throughout the community.
- Data-Driven Strategy: Gathering, evaluating, and using data to develop effective health programs.
- IRS Compliance: Meeting the IRS Form 990 Community Health Needs Assessment requirement.
- Employee Health Improvement: Leading internal health improvement programs for employees.
- Special Projects: Undertaking special assignments from the President/CEO.
Work Environment & Physical Demands:
- Environment: Office and community-based settings with potential for physical activity.
- Physical Requirements: Ability to lift up to 50 lbs, lead low to medium-impact exercise programs, and perform tasks like standing or walking for extended periods.
Mathematical Skills:
- Ability to calculate basic mathematical figures, such as percentages, ratios, and statistical analysis.
Qualifications:
- Experience: At least 2 years in a relevant field.
- Education: Bachelor's degree in healthcare, nursing, or a related field.
- Skills:
- Strong communication (oral and written).
- Public speaking and leadership abilities.
- Organizational and project management skills.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint, etc.).
- Data gathering and analysis.
