What are the responsibilities and job description for the Family Preservation Case Manager position at Clarvida - Illinois?
Our Family Preservation Case Manager meets with children involved with DCFS and their families. In this position, you will assess and identify the child and family needs, establish goals, and link them to the necessary services and/or treatment to move them toward a goal of safety and stability. You will meet with the children weekly at first, then bi-weekly to continue to assess and evaluate any needed services and goals, and document the child’s progress. You are provided regular and supportive supervision by the Case Management Supervisor, provide professional growth.

Does the following apply to you?
- One of the following:
- Bachelor’s Degree in a Human Service field (Social Work, Psychology, Counseling, etc.) and a minimum of two (2) years of related experience
- Master’s Degree in a Human Service field (Social Work, Psychology, Counseling, etc.)
- CWEL licensed
- Valid State Driver’s License and automobile insurance.
- If relocating to Illinois – IL driver’s license MUST be obtained within thirty (30) days of hire
