ESMI (Early Serious Mental Illness) CBRS/SEES - Community Based Services Professional

Our CBRS and Case Management providers deliver services to clients at their home and in the community focusing on their individual behavioral health and/or mental health needs. This position works with adolescent through adult clients who have behavioral issues, emotional disturbance, or severe and persistent mental health diagnoses. As a CBRS and Case Manager you will be responsible to maintain confidential documentation throughout the case including daily progress notes, treatment plan updates, and discharge summaries. In meeting with your clients weekly, you will focus on skill building, gaining access to community resources, and providing coping skills to foster independence and improve their quality of life.

ESMI (Early Serious Mental Illness) CBRS/SEES - Community Based Services are similar to regular CBRS/Case Management services with additional supervision requirements and SEES emphasis on vocational and educational goals.

ESMI CBRS And SEES Provider Duties Include

  • Maintain functions within the scope of practice as established by the contract developed through the Department of Health and Welfare regarding the ESMI program to include supported employment and educational services, with ongoing job coaching and support.
  • Interface routinely with Lead ESMI Clinician, Lead Supervisor, ESMI staff, clients and other key personnel as identified essential to client care.
  • Complete required initial and ongoing trainings, including CEU’s related to specific job duties, one of which must be ethics training annually
  • Attend weekly ESMI Staff Meeting
  • Provides service delivery in accordance with the objectives specified/authorized in the individualized treatment plan and in accordance with payer standards.

What We Are Looking For

  • Bachelor’s degree: Human Service field a plus (Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, Physical Therapy, Occupational Therapy, Therapeutic Recreation, Speech Language Pathology, Behavioral Sciences, Art Therapy, etc.)
  • Ability to pass background checks, including Enhanced Fingerprinting with ID DHW.
  • Valid driver’s license with reliable transportation and proof of car insurance
  • Experience utilizing electronic health records system a plus
