Summary
The Foster Care Support Specialist Coordinator assists the Urban Foster and Adoptive Support (FAS) Supervisor in the coordination of operations of the FAS program. This position ensures timeliness of reports and intervenes in problem area and provides consistency and knowledge of foster care licensing to ensure program integrity and compliance in the urban counties . This position will also carry a caseload.
Essential Duties And Responsibilities
- Provides support and consultation services to foster and adoptive families.
- Maintains a positive and collaborative relationship with the child’s placing agency.
- Monitors home for compliance with contract and state regulations.
- Assists in placement of foster care youth.
- Participates in the Child Family Team (CFT) meetings, attends court with the family, and participates in Team Decision Meeting s.
- Facilitates licensing inquiries in the foster homes and monitors Corrective Action compliance.
- Assists families with training requirements.
- Conducts in-person home visits per contract and foster care rule requirements.
- Maintains family licensing records in appropriate state and agency databases according to contract, HRT policy, and rule requirements.
- Provides crisis management.
- Submits renewal license applications prior to expiration.
- Submits license amendments, as needed.
- Writes adoption conversion home studies, as needed.
- Rotates on call services with other staff members.
- Attends training and events as requested by supervisor or other member of the management team.
- Maintains electronic Case Management (Client) and Provider Family Licensing Files for assigned providers in Extended Reach.
- Attends company related staff meetings, Team building events, Provider Recognition and other special events as needed.
- Acts as a second-level reviewer for home study submissions
- Provides support and training to other FAS team members as directed by the FAS Supervisor
- Covers team meetings as needed by the FAS supervisor
- Attends quarterly audits
- Assist with the coordination of renewals and amendment timely submittals
- Communicates regularly with HRT FAS Supervisor regarding Urban Program activities, needs, issues, successes, etc.

- Bachelors’ Degree in Social Work, Psychology or Human Services or related field and five (5) years’ experience with developmental disabilities, child welfare services and/or social services required or ten (10) years’ experience with developmental disabilities if no degree achieved.
- Ability to obtain and maintain a valid unrestricted Level One Fingerprint Clearance Card.
- Must meet the requirements to be a safe transporter and driver.
- Must possess a current Arizona State Driver’s License and have an acceptable driving record for the past three (3) years.
- Must have personal vehicle liability insurance and vehicle registration as required.
- Must be able to pass a Department of Economic Security ( DES ) Clearance with no substantiated reports.
- Must obtain and maintain Article 9 instructor certification.
- When required, the ability to pass a criminal background clearance check, drug screen, and if requested a physical exam.
- Must be able to work a flexible schedule.
- Must be able to maintain a high level of confidentiality.
- Must obtain and maintain CPR and First Aid certification.
