We have over 30 outpatient behavioral health locations nestled throughout our communities located in Kern County, Los Angeles County, Orange County, and San Diego County.
Clarvida is seeking an innovative and purpose driven individual to join our team as a Independent Living Skills Coach.
Summary
A Life Skills Coach has been a recipient of mental health services. Life Coaches are fully integrated team members who provide highly individualized services in the community and promote client self-determination and decision-making. Life Coaches also provide essential expertise and consultation to the entire team to promote a culture in which each client's point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help activities. Assists care coordinators in engaging consumers who come to the program, run groups and perform some clerical activities. Specific job responsibilities will vary depending on education and experience.
Pay : $19.25
Essential Duties / Responsibilities
- Carry out duties as a member of the team, under the direction of the team leader and the team psychiatrist.
- Be a member of an individual treatment team for a number of clients.
- Performs case / care management support functions to include obtaining background information and records on clients.
- Provides community resource linkages for clients and staff.
- Transport clients to accommodate community relationships. Use of personal car or vehicle is required.
- Assists clients with understanding and utilizing public services.
- Under direction completes clerical tasks such as filing, copying, putting together packets needed by care coordinators.
- Consults with supervisor and care coordinators regarding concerns with consumers attending groups or coming to clubhouse.
- Monitor cleanliness and tidiness of the clubhouse and waiting area of the program at all times.
- Maintains professional relationships with relevant agencies and service providers.
- Participates in staff and client meetings, and training activities as required.
- Facilitates some socializing groups at the clinic and in the community.
- May provide work-related supportive services, such as assistance securing necessary clothing and grooming supplies, wake-up calls, transportation.
- Meet monthly productivity requirement of 100 direct service hours (DSH).
- Perform mentoring, problem solving, and may provide encouragement on and off the job site within boundaries.
- Maintain and ensures HIPAA, ethical standards and professional boundaries with all staff and clients.
- Assist and support clients to carry out personal hygiene and grooming tasks.
- Provide practical help and mentoring, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain the necessities of daily living.
- Other duties assigned or necessary to support the program and / or the company.
