LPN-Tennessee Health Link Care Coordinator I

Description

We are seeking a Licensed Practical Nurse (LPN) to fill a specialized Medical Assistant position in tandem with our Nurse Practitioner (NP) appointments. This role is integral to supporting the behavioral health program and ensuring high-quality care for our patients.

Responsibilities include:

• Assisting with patient appointments under the supervision of the prescribing Nurse Practitioner.
• Completing necessary paperwork, forms, and assessments for patients receiving prescription medication.
• Maintaining detailed and accurate patient records.

Position Details:

• Licensure Requirement: Applicants must hold a valid LPN license.
• Caseload: This role will manage a reduced caseload (maximum of 40 patients) compared to a typical THL Care Coordinator (who manages approximately 80 patients) due to the additional focus on administrative and clinical support tasks.

Preferred:

• Licensed Practical Nurse or Bachelors in human services with field experience.
• CPR and First Aid certified, or be willing to obtain certification before working with clients; and other duties as assigned
